  • Memberships may be placed on a temporary hold for any of the following conditions: temporary job relocation, illness, seasonal change of residence, or travel.

    • This hold form must be completed and submitted 10 business days prior to the start date of the membership hold. Membership holds will be processed only once the Centennial Fitness Center receives a completed Temporary Membership Hold Form. This policy cannot be offered retroactively.
    • Temporary Membership Holds must be for a minimum of 30-days and are permanent and cannot be modified or cancelled. Membership passes on hold are invalid and members will not be allowed privileges associated with the membership pass. Members must pay the appropriate day fee when the membership is on hold.
    • 12 Month/Pay in Full Members: The membership obligation will be extended from the current expiration date to include the number of days the membership is to be on hold as to be determined by the Park Ridge Centennial Fitness Center.
    • EFT/CC Members: The membership obligation remains continuous. One payment will be suspended for every 30 days put on hold. Payments will resume as normal, on the first of the month, after the end of your hold date.
    • Membership holds will begin on the first of the month and conclude on the last day of the month.

  • Note that EFT holds must match the billing cycle; start of hold must be end of month and hold end date the start of the billing month.

  • By submitting this request I acknowledge I understand and agree to abide by the Park Ridge Park District Centennial Fitness Center Temporary Membership Hold Policies. I understand that this is a request and subject to approval by the Centennial Fitness Center in accordance with the Temporary Membership Hold Policies and Terms of Membership Agreement.
