    I understand that annual memberships are for a minimum of 12 active months and may only be cancelled within the first 12 active months for medical reasons or relocation with proof. All memberships cancelled within the first year are subject to a $35 termination fee. This applies to every member in the household. I understand that non-annual memberships are for the agreed upon membership term and cannot be cancelled, put on hold, transferred to another individual, or refunded; non- annual memberships will remain valid until the pass expiration date. I agree to abide by all policies and guidelines of the Park Ridge Park District Centennial Fitness Center and I acknowledge that the policies and procedures are subject to change without notice. I understand that the membership costs are subject to change with a 7- day notice and I agree to abide by these changes, even if it is during my obligatory period. I understand that other costs associated with the facility are subject to change without prior notice. I understand that my membership may be terminated by the Park Ridge Park District Centennial Fitness Center if I am in violation of policies or rules or conduct myself in a manner deemed inappropriate by management. I understand that I am bound by the terms of this agreement unless cancelled in writing within seven days after acceptance, or otherwise provided by law.

    EFT/Credit Card

    Our monthly annual membership plans, whether EFT or credit card, pays annual membership dues on a continuous monthly basis by deducting directly from your checking accounts, savings account, and credit card. A voided check must be provided at the time of registration. I understand that my EFT/Credit Card membership is for an initial 12-month period, after which the membership is continuous and requires no renewal. I understand my membership will automatically renew upon its expiration date and I am responsible for all accrued dues and other charges. I understand I am responsible for the entire amount due of this membership agreement.I understand it is my responsibility to keep updated account information on file with the Park Ridge Park District Centennial Fitness Center. Insufficient and past due payments will be assessed and will incur a thirty-five dollar ($35.00) service charge.

    Resignation

    As stated earlier, all annual memberships cannot be cancelled within the first 12 active months unless it is due to medical reasons or proof of relocation. All annual memberships cancelled within the first 12 months are subject to a $35 cancellation fee unless proof of relocation or medical reasons. After the first 12 months, it is agreed that an annual member may resign from the Park Ridge Park District Centennial Fitness Center by giving seven (7) days written notice of intent to cancel the membership. All resignations must complete a Cancellation Request and submit it to the Centennial Fitness Center. Cancellation requests are accepted via drop-off, fax, mail or email. All accrued dues and other charges for which the member may be liable to the date of resignation and all membership cards are due upon the effective date of resignation. I understand non-annual memberships are unable to be cancelled and are non-refundable. Non-annual memberships are valid until the agreed upon expiration date.

    Temporary Membership Hold

    Annual Memberships may be placed on a temporary hold. This policy cannot be offered retroactively and an official hold form must be completed and received by the Park Ridge Park District Centennial Fitness Center prior to the start date of the membership hold. Hold forms are accepted via drop-off, fax, mail or email. Memberships must be put on hold for a minimum of 1 month and a maximum of 6 months. A membership hold inactivates a membership for the duration of the hold.

    Equipment/Property Damage & Personal Property

    I agree to use all equipment and property of the Park Ridge Park District Centennial Fitness Center for its intended use. I understand that if Park District property and/or equipment is damaged due to a direct result of my use that I am responsible for all costs incurred by the Park Ridge Park District to fix or replace the damaged property/equipment. I agree that the Park Ridge Park District and the Centennial Fitness Center are not responsible for stolen, damaged, or lost personal property.

    Facility Closures

    I understand that the Centennial Fitness Center may close due to maintenance or severe or hazardous weather conditions for up to two weeks in a calendar year. I understand that this is built into my membership cost and my membership will not be extended, prorated, or refunded due to facility closures.

    Membership Agreement

    I acknowledge that before I signed this agreement, I had the reasonable opportunity to examine it and agree to abide by the Park Ridge Park District Centennial Fitness Center's policies and procedures. I understand that the Centennial Fitness Center is not obligated to give me a copy of the Centennial Fitness Center Handbook and that I may receive a complete copy upon request. I am aware that I may access the Centennial Fitness Center handbook at any time by visiting the Park Ridge Park District website at www.prparks.org. I agree to all information set forth in the Centennial Fitness Center Handbook and understand that it may be updated at any time as deemed appropriate by management. I acknowledge that I have been given, agree to and understand a waiver and release, and that this agreement is not valid unless accompanied by a signed waiver and release.

    Upgrading Membership (Short Term Only)

    Short-term membership is a non-annual membership. Transfer to an annual membership will be accepted provided written application for an annual is submitted to the Centennial Fitness Center prior to the short-term membership expiration date.

  • Centennial Fitness Center Waiver & Release

  • IMPORTANT INFORMATION: The Park Ridge Park District is committed to conducting its recreation programs and activities in a safe manner, providing safe aquatic facilities and programs, and holds the safety of participants in high regard. The Park Ridge Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participant's safety. However, participants and parents/guardians of minors registering for this program must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs.

    You are solely responsible for determining if you or your minor is physically fit and/or adequately skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, suffers from an underlining medical condition, takes medication, smokes cigarettes, has a family history of coronary disease, or has recently suffered an illness, injury or impairment, to consult a physician before undertaking any fitness center activity, or any physical/aquatic activity.

    Please understand and recognize that lifeguards are not responsible for providing supervision or assessing your swimming skills or that of your minor child; rather, lifeguards are responsible for enforcing safety rules and responding to emergencies. Adult pool pass holders and parents of minor pool pass holders are solely responsible for supervision of any and all activities contemplated by this agreement. Additionally, children 8 years of age and younger must be supervised at all times by a responsible person, 16 years of age or older. NEVER LEAVE ANY CHILD WITH POOR SWIMMING SKILLS OR 8 YEARS OF AGE AND YOUNGER UNACCOMPANIED BY A PARENT OR RESPONSIBLE PERSON, 16 YEARS OF AGE OR OLDER.

    WARNING OF RISK: Aerobic and other fitness activities such as passive/resistive weight training, use of stair machines, jogging, free weights, and other training devices are intended to challenge and engage the physical, mental and emotional resources of the participant. Despite careful and proper preparation, instruction, medical advice, conditioning, and equipment, there is still a risk of serious injury. All hazards and dangers cannot be foreseen. Depending on the particular activity, certain risks, dangers and injuries due to overexertion, improper technique, ignoring safety precautions, failing to follow instructions, slips and falls, unfamiliarity with the equipment and/or exercise, equipment failure, failure in supervision/instruction, premises defects and other risks inherent to the particular activity exist. In this regard, it is impossible for the Park Ridge Park District to guarantee absolute safety.

    Dependent upon a person's physical condition, age and/or skill level, aerobics and fitness activities can involve a substantial risk of the following types of injuries. This list is by no means complete, but includes some of the more common ones: 1. Heart attack, stroke and circulatory problems 4. Shin splints 2. Bone and joint injuries 5. Muscle strain and other muscle injuries 3. Back and neck injury 6. Foot problems.

    Swimming and other aquatic activities challenge and engage the physical, mental and emotional resources of each participant. However, despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury, including but not limited to drowning, head/brain injury, and spinal cord injury. Understandably, not all hazards and dangers can be foreseen. The very nature of swimming and aquatic activities are hazardous and risky, including but not limited to fatigue and overexertion, poor swimming skills, failing to avoid dangerous areas, failing to follow rules and regulations, failure of lifeguards to locate victims and/or delay in emergency response time, horseplay, diving or cannon-balling into shallow water and striking the bottom or side of the pool or waterslide, inadequate supervision or instruction, lack of conditioning, becoming disoriented, striking other swimmers, striking one's head on the bottom, slip and falls on the deck or within the locker facility, chemical exposure and all other circumstances inherent to aquatic activities. In this regard, it must be recognized that it is impossible for the Park District to guarantee absolute safety.

    WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK: Please read this form carefully and be aware that in signing up and participating in this program/activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with this program/activity (including transportation services and vehicle operations, when provided) and with the use of the Park District aquatic facilities and programs.

    I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity and to patrons of aquatic facilities, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of participating in any and all activities and programs connected with or associated with this Pass. I agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in this program/activity against the Park Ridge Park District, including its officials, agents, volunteers and employees. I further recognize and agree that lifeguards and other aquatic staff are not responsible for supervising my activities or the activities of my minor child(ren) and I agree that I am solely responsible for supervising my minor children and/or assessing whether my children are physically fit and/or adequately skilled for aquatic activities. I additionally agree to supervise any children ages 8 and under at all times. I further agree to waive and relinquish all claims I, or my minor child/ward may have (or accrue to me or my child/ward) as a result of use of the Park District's aquatic facilities and programs against the Park District, including its officials, agents, volunteers and employees (hereinafter collectively referred as "District" I do hereby fully release and forever discharge the District from any and all claims for injuries, damages or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in any way associated with use of the Park Ridge Park District's aquatic facilities and programs.

    I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering on-line or via fax, my on-line or facsimile signature shall substitute for and have the same legal effect as an original form signature.

  • I acknowledge that I am selecting the monthly bank draft (EFT) method for payment of my Centennial Fitness Center Membership dues. I hereby request that my monthly dues be charged directly to the checking/savings account at my bank. So that the Park District may implement a monthly draft to my checking/savings or credit card account, I agree to provide the Park District with a voided check that shows my routing number and account number. I understand that the monthly draft will be charged to my account the first day of every month and that my bank should provide me with a receipt of payment. I further understand that should my monthly bank draft be returned due to insufficient funds (NSF) that I will be responsible to pay the Park District a NSF fee of $35.00 and that the following month my account will be charged the past due and current amounts owed. It is further noted that if there is any procedural delay in the implementation of the bank draft, I will be back-billed for the months for which billing is delayed. THIS MONTHLY BANK DRAFT WILL REMAIN IN EFFECT UNTIL THE PARK DISTRICT RECEIVES A WRITTEN NOTICE OF CANCELLATION. The written notice of cancellation must be received by the last day of the month or charges will be applied to your account for the following month. Please allow 30 days for processing.

  • Welcome to the Park Ridge Park District Centennial Fitness Center!

    In order to ensure you are provided with the best service possible and that you are ready to enjoy the full benefits of your membership, please take a few moments to complete this checklist.

  • As a new member of the Park Ridge Park District Centennial Fitness Center, I have been explained and understand the following:

    • Included in My Membership! My membership includes access to the fitness center, indoor running/walking track, Schumacher Natatorium (indoor aquatic center), open gym times, use of the racquetball courts and towel service. Wait, there's more! I also get limited access to Centennial outdoor pool over the summer (lap swim times) and reduced rates on drop-in group fitness and personal training packages.
    • Private Lockers I can add-on to my membership to include access to the private, adult-only, locker rooms. This includes a permanent locker, cable, television, and some complimentary toiletries. Additional fee required.
    • Group Fitness Programs Join the group for any of these exhilarating and refreshing workouts. Whether you're new to fitness or an avid fitness enthusiast, the Centennial Fitness Center will surely have a class for you. Additional fee required.
    • Personal Training Services The Centennial Fitness Center has a team of personal trainers ready to motivate your workouts. Additional fee required.
    • Fitness Area Age Restrictions Participants under 18 years of age must have a parental waiver on file prior to accessing the facility. Participants ages 13-15 must complete a teen fitness orientation prior to using the fitness center. Participants under 13 years of age are not allowed in the fitness area or track.
    • Equipment & Property Use I agree to use all equipment and property of the Park Ridge Park District Centennial Fitness Center for its intended use and I understand that I am responsible for any damage that I may cause.
    • Easy & Convenient Membership Holds I can place my annual membership on hold a minimum of 1 month and a maximum of 6 months by completing a membership hold form 10 business days prior to the 1st of the month.
    • Resignation Notification Period & EFT/Credit Card Cancellation I understand that the Centennial Fitness Center has a 10-day notification period when cancelling my membership and that all resignations must be submitted in writing. I understand that if I pay through my membership through EFT that I must submit my cancellation request 10 days prior to the 1st of the month to avoid any additional charges.
