Liability, Behavior, and Health Information
The paragraphs below outline behavior expectations and permissions given to the Montana Conference of Seventh-day Adventists in providing health and emergency care. By checking each box below, you indicate your consent and understanding of the expectations. This is an entrance requirement for Summer Camp.
Liability Waver

The above named camper has my permission to attend Summer Camp located at Camp Paxson in Seeley Lake, Montana. It is understood that I will not hold the Montana Conference, the churches, or their personnel responsible in case of accident or injury beyond that coverage provided by the student accident insurance program of the Montana Conference. Although reasonable steps have been taken to provide appropriate equipment and skilled instructors, the activities are not without risk. Certain risks are inherent in each of the activities and cannot be eliminated without destroying the unique character of the activity and can be the cause of loss or damage to equipment or accidental injury, illness, or in extreme cases, permanent trauma or death. The camper's participation in the activities is purely voluntary, and noone is forcing the camper to participate. Each activity may require a degree of skill and knowledge. Each camper has responsibilities as a participant. As the parent/guardian, I certify that my camper has no health or physical condition that will interfere with the activities or is more susceptible to injury than the average person.

Cell Phones & Electronics Policy

Please note that our policy has changed. We have a NO phone or electronic device police for campers. At registration we will ask that the parent/guardian will take the campers phone/electronic device with them. If the camper is riding the bus this will happen when the camper checks in for their ride to camp. If your campers is found with a phone/electronic device after registration it will be confiscated and returned at checkout. We are NOT responsible for any lost, damaged or stolen phones or electronics. If you need to contact your child at camp please call 406-351-0811.
Photo & Video Consent
Periodically, campers participating in summer camp activities may be photographed, or filmed. We need your permission for your child to be photographed or filmed as it applies only to promotiing the Camp Paxson program. By checking this box and signing below you give permission for your child to be photographed or filmed.

Consent to Treat

We, the undersigned parents or guardian of the above named camper, a minor, do hereby consent to any x-ray, examination, anesthetic, medical or surgical diagnosis or treatment and hospital service that maybe rendered to said minor under the general or special instructions of the named medical clinic or my child’s doctor, or any physician the school or organization may call, whether such diagnosis or treatment is rendered at the office of said physician or at a licensed hospital. It is understood that reasonably effort will be made to contact the doctor listed above before any other physician is called by the school or other organization.

Consent to Treat

This consent shall remain in continuous effect until revoked in writing and delivered to the physician named above or to the camp or organization entrusted with the custody of said minor. We hereby authorize any hospital, physician, or other person who has attended or examined the minor to furnish the General Conference Insurance Service, or its representative, any and all information with respect to any illness, medical history, consultation, prescriptions, or treatment, and copies of all hospital medical records. A photo copy of this authorization shall be considered as effective and valid as the original.
