    How to Use This Assessment

    1. Find a quiet space and give yourself 20-30 uninterrupted minutes
    2. Be honest—this is for your eyes only
    3. Rate each area using the 1-10 scale based on the past 2-4 weeks
    4. Calculate your scores
    5. Read your results and review your personalized interpretation and action steps
    6. Take action or reach out for support

    For each question, rate your experience on a scale of 1-10, based on the past 2-4 weeks:

    • 1-3: Minimal Impact (rarely experience this or doesn't significantly affect your daily life)
    • 4-6: Moderate Impact (regularly experience this, noticeable impact on wellbeing)
    • 7-8: Significant Impact (frequent experience, seriously affecting your quality of life)
    • 9-10: Severe Impact (constant experience, major disruption to daily functioning)

    Tips for Accurate Assessment:

    • Go with your first instinct
    • Focus on the past 2-4 weeks
    • Be honest (no one else will see your answers)
    • If a question doesn't apply, mark it as "1"
    • No right or wrong answers
