Take Charge Chronic Disease Self Management Program: Spring 2026
Take Charge: Chronic Disease Self-Management Program is an 8-week workshop for participants who are in various stages of Chronic Kidney Disease. The first six weeks focus on overall healthy living, with two additional sessions that cover kidney specific content. The 8 week program will be held on Monday mornings between 10:00 am - 12:30 pm central.

This class will be held virtually. You will receive the Zoom link by e-mail before the first week of class. Please plan to attend all 8 sessions.
