Pet Adoption Application

Apply to adopt a cat, dog, or small animal. Please provide your information and tell us about your home and experience with pets.
In order to be considered as an adopter you must:
*Be 18 years of age or older
*Have identification showing your 18 years of age or older
*Have identification showing your present address
*Have the knowledge and consent of your landlord if needed
*Be able and willing to provide a loving, safe and peaceful environment for this pet
*Be financially stable and willing to provide any necessary medical care address
The following questionnaire has been designed to aid both you and Walker's Way Rescue in deciding if you and/or your family are adequately prepared to assume the responsibility of pet ownership. Please be sure to ANSWER ALL QUESTIONS and feel free to add your own comments. If a question does not apply write N/A. If the application is left blank in some area's we will automaticly decline application.

Do your current pets get along with other animals?
Where will your new pet sleep at night? Explain what you would do at night for a dog and where will the cat go at night.
How many hours per day on average will the pet be alone? This also does not affect the decision of adoption. A animal in a home is better than a dog in a shelter.

Does your yard have a fence? Yes or No. If yes, what type of fence? Not having a fence does not mean you are automatically denied from adopting. We feel STRONGLY not to adopt dogs to homes with electric fences without a discussion about them first. The only encourages the safety of our dogs.
Will you ever have your dog on a tie out or in a dog house outside?
How many people in your household?
Is everyone in your household in agreement with adopting a pet? We highly encourage the whole family meet the animals they are thinking of adopting. A happy animal is loved by everyone in the household. Meets and greets are a must.

We will NOT adopt to anyone that will declaw a kitten or cat! We feel strongly that this is inhumane and it is illegal in RI as of September 1st 2025. Do you agree?
