  • Introduction

Midlife isn’t just “getting older.” It’s a time when identity, relationships, hormones, work, health, and purpose can all shift at once. That can leave you feeling anything from quietly restless to completely unraveled.

This short questionnaire is designed to help you understand where you are in your midlife transition right now — not as a diagnosis, but as a compassionate snapshot of your inner world.

You’ll answer 3 introductory questions then 10 questions about how you feel in yourself, your relationships, your emotions, your body, and your sense of direction. At the end, you’ll see which stage you’re most aligned with:

    • Awakening – “Something’s shifting; I want more alignment.”
    • Transition – “I’m between an old life and a new one.”
    • Unraveling & Rebuilding – “I feel like I’m falling apart… and don’t yet see what’s next.”

There are no right or wrong answers. Just choose what feels most true today.

When you’re ready, take a breath, soften your shoulders, and begin.
