Ace’s Place Ornament Order Form

Your design is created using reference photos of your pet and will be transformed into line art engraved into a 3.5-4” wooden round, sealed twice on each side. Please submit one form per ornament, multiple ornaments will still be delivered together.
Each ornament is $20, with free shipping. Payment can be made via Venmo, PayPal, Zelle, or on our website which accepts any card. All orders are started after both payment and this form are submitted.

Side 1 will contain a line art portrait of your pet, designed based on photos provided, with their name below the image in your desired font. Portraits will be neck-up without any collar/harness/leash unless otherwise specified.
