Waiver and Consent
In acceptance of the right to participate in the above event, I hereby release Hugo Alumni Association and Hugo Public Schools, its community partners, successors, assignees, licensees, sponsors, and any other entity connected Hugo Alumni Association and Hugo Public Schools for any and all known or unknown damages, injuries, losses, and /or claims whatsoever, that may occur to myself or property with respect to my participation during the listed event.
Media Release
I understand that Hugo Alumni Association and Hugo Public Schools produces promotional material about their program. I understand that I may be included in video tape or photography taken during this event. I hereby grant Hugo Alumni Association and Hugo Public Schools, its community partners, successors, assignees, licensees, sponsors, any television networks and all other commercial exhibitors the exclusive right to photograph and/or video tape my name, face, likeness, voice and appearance as part of the event, and in advertising and promotion of the event, without reservation or limitation. In granting this license, I understand that Hugo Alumni Association and Hugo Public Schools is under no obligation to exercise any of its rights, licenses and privileges herein granted.
