Injury and medical waiver

On behalf of my team, I acknowledge that basketball can be dangerous and that participation could lead to bodily injury or death. In consideration of this, on behalf of each player in my team, I hereby assume the risk of the players above and accept full responsibility to pay expenses for medical care that may arise from participation. I acknowledge and assume the risks inherent in this event and hereby relieve RISE 3x3 Basketball and any partners of any and all liability. RISE 3x3 and its partners will not be responsble for loss or theft of money or personal articles.

Comment to adhering to Covid-19/Flu Rules

On behalf of y team, we will contact the event organisers in advance of the tournament if any players show signs of illness relating to Covid-19 or Flu (or anything else considered dangerous and/or contagious).

Consent to Imagery

On behalf of my team, we consent to the use of imagery of anyone on my team (including myself) to be used by any media present at the evenrt, including (but not limited to) my name, voice, likeness and actions. This can be used for promotional, marketing or any other form of content that is deemed useful to RISE 3x3 and associated media, partners and businesses. I grant RISE 3x3, media and associated partners and businesses perpetual, worldwide licence for any photoraphyor video recordings of our team members.
