• Terms & Conditions

    • All health history information, current medications and name / contact information of the patient’s primary care physician must be provided by completing the Health History Form as part of the scheduling process. All information will be kept private.
    • Appointments fees must paid for in full prior to selecting a schedule time. All appointment and associated fees are non-refundable. If an appointment time is changed within 48 hours or more prior to the appointment time, all pre-paid fees will be applied as a credit to the patient’s account and will remain available to apply towards a subsequent future appointment within 3 months of the previously cancelled appointment. If after 3 months the account credit is not utilized, the credit will be fully forfeited to 2nd Opinion Dental and any subsequent appointments then scheduled will incur new fees.  Cancellation of any appointment within less than 48 hours from the upcoming appointment time will result in a complete forfeiture to 2nd Opinion Dental of all appointment and associated fees.
    • Patients that wish to submit for reimbursement from their dental insurance will do so themselves and will be given the appropriate information and potential service codes that may be required for them to submit to their insurance.  2nd Opinion Dental will not be held financially responsible for any service(s) or service code(s) that may be denied by any patient's dental insurance provider, as ALL services to be provided by 2nd Opinion Dental must be paid for in full prior to scheduling an appointment.
    • Any and all dental diagnostic and treatment planning information will be provided as accurately as is possible. A written copy of all pertinent information will be furnished to the patient shortly after the completion of their appointment. 2nd Opinion Dental does not hold responsibility and will not be held liable due to any patients’ changes in diagnosis over time and/ or due to their lack of pursuing timely dental care. 2nd Opinion Dental will also not be responsible or held liable for any diagnoses and / or treatments that are subsequently made by their treating dentists, regardless of time or outcome related to the original findings and recommendations made by 2nd Opinion Dental.
