Please complete the PADI - Medical questionnaire by answering EVERY question on pages 1 and 2 (check all the boxes). Regardless of your answers on the PADI - Medical form, every diver is required to have a doctor's signature on page 3 at the request of IFS even if it says you do not need one on the form.

(4) Dive Insurance

Please upload a copy of your dive insurance. We recommend purchasing the Regular Individual Membership through Divers Alert Network (DAN). Additional insurance packages through DAN are optional. If you purchase the DAN membership, please submit a screenshot of your membership ID card. *Anyone who is participating in DSD at the field station is required to have dive insurance.
