Please complete the PADI - Medical questionnaire by answering EVERY question on pages 1 and 2 (check all the boxes). Regardless of your answers on the PADI - Medical form, every diver is required to have a doctor's signature on page 3 at the request of IFS even if it says you do not need one on the form.

(4) Dive Insurance

Please upload a copy of your dive insurance. We recommend purchasing the Regular Individual Membership through Divers Alert Network (DAN). Additional insurance packages through DAN are optional. Please submit a screenshot of proof of Dive Insurance. *Anyone who is participating in DSD at the field station is required to have dive insurance.
