Loaner Vehicle Form

A. DRIVER REQUIREMENTS & OPERATING STANDARDS

Valid License (Illinois)
I certify that I hold a valid Illinois driver’s license (or a valid out-of-state license legally recognized in Illinois) and am fully authorized to operate a motor vehicle under Illinois law.
Authorized Driver Only
I am the only authorized driver of the loaner vehicle.
No additional drivers, and no one under age 25 may operate the vehicle.
Prohibited Uses
I agree NOT to use the vehicle for the following

-racing or track use
-towing or hauling
-commercial or delivery purposes
-driving under the influence of alcohol or drugs
-off-road use
-reckless, aggressive, or negligent driving
Care of Vehicle & Responsibility for Tickets
I will exercise reasonable care for the vehicle and agree to pay any traffic, toll, parking, or other violations incurred during the loaner period.
Mechanical Issues
If any warning light, smoke, noise, or abnormal condition occurs, I will safely pull over and immediately contact the Company.
Failure to do so may make me financially responsible for resulting damage.

 

B. INSURANCE & LIABILITY

Required Insurance Coverage (Illinois)
I certify that my vehicle insurance meets or exceeds the Illinois minimums:

$25,000 bodily injury per person
$50,000 bodily injury per accident
$20,000 property damage
The Company may require proof of insurance before releasing the loaner vehicle.
Primary Insurance
I agree that MY insurance policy provides primary coverage for any damages, losses, or liabilities arising from my use or possession of the loaner vehicle.
Damage, Loss, Collision, Theft
I am financially responsible for:

all physical damage to the loaner vehicle, regardless of fault
deductible(s) on my insurance policy
loss of use (daily rental value while the car is out of service)
diminished value
towing or storage fees
full replacement value if the vehicle is totaled or stolen
Indemnification
I agree to indemnify, defend, and hold harmless Aesthetic Styling, its owners, employees, contractors, and affiliates from all claims, losses, damages, or liabilities arising from my possession or use of the loaner vehicle.
Personal Property Disclaimer
The Company is not responsible for any personal property left in the loaner vehicle.

 

C. TERMS, RETURN, AND DISPUTES

Return Condition
I agree to return the vehicle:
in the same condition it was provided (normal wear excepted),
with the same fuel level, unless the Company specifies otherwise.
Jurisdiction (Illinois)
This Agreement is governed by Illinois law.
Any disputes must be brought exclusively in the Illinois state court located in DuPage County, the jurisdiction of Aesthetic Styling.
Acknowledgment
I have read and fully understand this Agreement.
I acknowledge that this is a courtesy vehicle, not a rental, and is provided solely as a convenience while Aesthetic Styling services my vehicle.

AESTHETIC STYLING – LOANER VEHICLE RULES & GUIDELINES

These rules and guidelines are designed to ensure that the loaner vehicle is treated with the same level of care that Aesthetic Styling provides to your vehicle. By accepting the loaner vehicle, the Customer agrees to the following terms:

1. Fuel Level Policy
The vehicle must be returned with the same fuel level it had at checkout.
Any shortage will be charged at $25 per quarter-tank missing.

2. Wheel & Curb Rash Damage
Any curb rash, scraping, or damage to a wheel results in a $250 per wheel repair charge.

3. Paint Protection Film (PPF) Damage
If any PPF panels on the loaner vehicle are scratched, torn, lifted, punctured, or otherwise damaged:

The Customer will be charged the current wholesale replacement cost for the panel, at a set price of $395 per panel damaged.

4. Mileage Restrictions & Road Trip Policy
Loaner vehicles are not permitted to be used for road trips or extended travel.
A daily mileage allowance of 50 miles per day applies unless otherwise stated in writing.
Excess mileage can be billed at $0.30 per mile.

5. Interior & Cleanliness Policy
No smoking of any kind inside the vehicle. Smoking fee: $250.
No pets are allowed inside the loaner vehicle.
Excessive dirt, stains, spills, or interior mess requiring cleaning will be billed at $50–$150, depending on severity.

6. Loss of Use
If the loaner vehicle is damaged and cannot be used during repairs:

Customer agrees to pay a loss-of-use fee of $50 per day, or the Company’s standard rental rate, whichever is greater.

7. Diminished Value
If the loaner vehicle suffers damage that results in reduced market value even after repairs:

Customer is responsible for paying the diminished value amount as determined by industry-standard valuation methods.

8. Unauthorized Drivers
Only the Customer listed on the agreement may operate the loaner vehicle.
If an unauthorized driver operates the vehicle, the Customer assumes full financial responsibility for ALL damages, including those not covered by insurance.

9. General Respect & Care
This loaner vehicle is offered as a courtesy while Aesthetic Styling services your vehicle.
We ask that customers treat our vehicle with the same care and respect that we provide to yours.

10. Toll Usage & I-PASS Responsibility
The Customer is fully responsible for all I-PASS tolls, toll road usage fees, and any associated fines or violations incurred during the loaner period. If unpaid tolls or violations are billed to Aesthetic Styling, the Customer agrees to reimburse the full amount plus any administrative or penalty fees charged by the Illinois Tollway or third-party billing agencies.
