After submitting your household financial information in Step 1, please complete this section for each student in your family who is applying for camp support.

On behalf of myself, my household members, and my child, I give permission for my child to attend classes, camps, and programs offered by Little Blue Theatre Company (“Little Blue”). I understand that participation in these programs is voluntary.

I acknowledge that, while Little Blue takes reasonable steps to reduce the spread of illness, it cannot eliminate the risk of exposure to communicable diseases (including COVID-19, strep, and others). I agree to review and follow any health and safety guidelines provided by Little Blue to help protect my child, other participants, and staff.

In consideration of my child’s participation, I release and hold harmless Little Blue Theatre Company, its employees, teaching artists, contractors, representatives, successors, and assigns from any liability for accident, injury, or illness that may occur during participation in these programs, except in cases of gross negligence or willful misconduct.

I authorize Little Blue staff and program leaders to seek emergency medical treatment for my child if necessary.

Photographs and video recordings of my child(ren) taken by Little Blue may be used for sharing privately with enrolled families. They may also be used by Little Blue for promotional purposes, including brochures, advertising, social media, and the Little Blue website. These images will not be shared with or licensed to any outside organizations without my additional written consent.
