    • I certify that this health history and all information on it is complete and accurate and that I and/or my student is physically and emotionally fit to participate in an extended offshore sailing voyage. In the event of a medical emergency, I hereby authorize Sailing Ships Maine, its doctor(s), the ship’s captain and/or the medical officer to administer emergency medical treatment; to secure proper treatment and hospitalization; and to order injections, anesthesia, and/or surgery for me. I give permission for Sailing Ships Maine staff to share information from this form if needed for medical purposes. I understand that I am responsible for notifying Sailing Ships Maine of any injury, illness, other medical change to the medical information here provided. 

      For all parents and guardians: Signing below also acknowledges authorization to provide any emergency medical treatment for your child/ward.

    • Participant Release Form 
    • It is important to Sailing Ships Maine that you understand the nature of its programs and that you are informed regarding certain issues, including risks and limitations of liability. Your signature below reflects certain understandings and, with respect to adult participants and parents and guardians, certain agreements. The school/organization, parent, or guardian makes these agreements for himself or herself individually and on behalf of and for the minor participant. “I” and other first-person references below refer to the Participant and/or Guardian and to the school/organization or parent or guardian unless indicated otherwise. The Participant and/or Guardian do hereby freely execute this Release under the following terms: 

      1. Waiver and Release: By signing this Release, the Participant and/or Guardian hereby release and hold harmless Sailing Ships Maine and its successors and assigns, its directors, employees and staff from any and all liability, claims, and demands of whatever kind or nature which arise or may hereafter arise as a result of the Participant’s participation in the Voyage. The Participant and/or Guardian understand that this Release discharges Sailing Ships Maine, its directors, employees, and staff from any liability or claim that the Participant may have against Sailing Ships Maine regarding any claim or demand that may result from the Participant participating in the Voyage. 


      2. Medical Treatment: The Participant and/or Guardian release Sailing Ships Maine from any claim whatsoever which arises or may hereafter arise as a result of any first aid, treatment, or service rendered in connection with the Participant’s participation in the Voyage or with the decision by any representative of Sailing Ships Maine to exercise the power to consent to medical or dental treatment. 

      3. Assumption of Risk: The Participant and/or Guardian understand that the Voyage may include activities which may be hazardous to the Participant. Therefore, the Participant and/or Guardian assume any and all risk of any kind that may arise out of the Participant’s participation in the Voyage. 

      4. Media Release: By signing this Release, the Participant and/or Guardian also transfer unto Sailing Ships Maine all right, title and interest in any and all photographic images, video, and audio recordings made during the program including, but not limited to, any royalty benefits or other proceeds that could be received from such photographs or recordings. The Participant and Guardian also allow Sailing Ships Maine to use photographs and video taken during the program for the benefit of Sailing Ships Maine.

      5. The Participant and/or Guardian agree that in the event that any clause or provision of this Release shall be held to be invalid by a court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release, which shall continue to be enforceable. 

      6. Privacy Policy: All information given on this form is used by Sailing Ships Maine solely as a means of maintaining accurate records and conveying up-to-date information with all participants of our affiliate. All information is private and in no way will we disclose, share or sell personal information to any parties outside of the organization. 

      I, the undersigned, affirm that this agreement supersedes any and all previous oral or written promises or agreement. I understand that this is the entire agreement between me and Sailing Ships Maine and cannot be modified or changed in any way by representations or statements by any agent or employee of Sailing Ships Maine. This agreement may only be amended by a written document duly executed by all parties. The Participant and/or Guardian have executed this Release as of the day and year written below.

    • WE ARE A ONE STRIKE PROGRAM

      Failure to abide by the rules and regulations of the program will result in removal from the program at the earliest prudent opportunity at the Participant’s or their parents’ or guardian’s expense. The one-strike program relates to, but is not restricted to, the use and/or possession of illicit drugs, alcohol, and/or tobacco products. 

      Sailing Ships Maine also will not tolerate harassment of any kind, but particularly harassment forbidden by law and based on race, religion, gender, sexual orientation, age, national origin, race, marital status, and/or physical or mental disability. Social behaviors that put oneself and/or others at risk or jeopardize the integrity of the ship and voyage will result in immediate dismissal from the program. 

      Please understand that the severity of this response is due to the following: 

      ● Your behavior plays a direct role in the lives of fellow shipmates.  

      ● Trust is critical in the safe and successful operation of this program. 

      ● Depending on the program, you may be making port stops in foreign countries where the legal response to certain violations will result in incarceration. 

    • Acknowledgment of the Code of Conduct 

      I understand that Sailing Ships Maine operates under a one-strike policy. Violation of the rules and regulations, related but not limited to illicit drugs, alcohol, tobacco and/or social violations, will result in immediate dismissal from the program at the earliest prudent opportunity. This rule structure is in effect throughout the duration of the program, including transportation to and from the ship.
