Informed Consent and Acknowledgement

I give permission for my child (or ward) to participate in any and all activities prepared by the RFA camps. I understand the nature of the programs in which they will be participating, and the inherent risks in participating in these programs. I understand that campers registered in RFA’s summer camp program may participate in various neighborhood walks, outings, or outdoor filming conducted in the city of Missoula and I authorize my child or ward to participate. I hereby waive any claim against RFA or its authorities for any injury suffered by my child in connection with the walk or outing. Parents will receive notice prior to campers traveling to a secondary location outside of the Roxy Theater and its surrounding area.

Medical Release and Authorization

In the event of a medical emergency occurring to my child (or ward), I hereby authorize the RFA to act in loco parentis to my child (or ward). RFA may authorize all necessary medical and/or surgical procedures that are deemed vital for the health of my child (or ward). I understand that every effort will be made to contact me and that medical or surgical procedures will be implemented only in the event that I cannot be contacted immediately.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Compliance

I have read, understand and will comply with all registration, tuition, cancellation policies, rules, and procedures and have read and agree to accept the above obligations and responsibilities. If I have questions or concerns, I will reach out to RFA or The Roxy Theater via phone or email.

Photo/Video/Audio Release

I grant to RFA and all those acting with its authority the unrestricted right to use my child's/children's/ward's photos, videos, and audio as captured during the camp activities to copy, modify, create derivatives and otherwise use an image and/or audio in any medium; and use and permit to be used their name, whether in original or modified form, in connection with the images and/or audio as RFA may choose. I waive all rights and release and discharge RFA from, and will neither sue nor bring any proceeding against RFA for, any claim, demand or cause of action, whether now known or unknown, for defamation, invasion of right to privacy, publicity or personality or any similar matter, based upon or relating to the use and exploitation of the images and/or audio.

*We are flexible! Please let us know if you have any issues or concerns with the release or use of your child's image/audio.

Each camp has limited capacity and is close to filling up. If an option isn't able to be selected, the camp is full. Please email us at learn@theroxytheater.org to get on the waitlist!
