The purpose of this form is to determine who will be continuing at Zion Lutheran School for the rest of the 2025-2026 school year, for our 2026 Day Camp Program, and for the 2026-2027 school year. Please be sure to complete one form for each child and Submit Form (s) by Monday, February 9, 2026
