  • Participants are required to pay weekly rate, and a once-a-month supply fee.

    Rent is due every Friday by 10:00pm to the Owner Wade or House Manager.

    Acceptable forms of payment are as follows. Cash, or Zelle at no fee, cashapp or venmo has an additional 3% processing fee that needs to be added. Or on the AZSOBER.COM website which has 3% fee already added.

    Plugged In Recovery LLC does not authorize any sort of refunds.

    Seven-Day Written Notice

    Participants may receive a Seven-Day Written Notice under the following circumstances:

    Nonpayment of Fees, Charges, or Deposits: Failure to pay required program fees, charges, or deposits by the agreed-upon due date.

    The notice will outline the specific amount owed, the due date of the payment, and the required actions to resolve the issue within seven calendar days. Failure to address the matter within the specified time frame may result in further actions, including potential termination of participation in the program.

    Participants are encouraged to communicate with staff immediately if they encounter financial difficulties to explore potential solutions.

    Room Abandonment

    If at any time during the term of this Agreement, the Participant abandons the room for a period of two (2) consecutive days, Plugged In Recovery LLC, without liability for damages, may remove personal property and may also dispose of any of Participant’s abandoned personal property if arrangements are not made within 72 hours from the date of the abandonment.

    If Abandonment occurs, any prepaid fees will be forfeited.

  • Room Description and Amenities

  • As a participant of Plugged In Recovery, you will be provided with a comfortable and supportive living space to aid your recovery journey. Room furnishings and amenities are allocated based on shared or private room arrangements as follows:

    Shared Rooms

    Bed and Bedding

    Each participant is provided with an individual twin or full-size bed, mattress protector, pillow, and linens (including a fitted sheet, flat sheet, and blanket).

    Shared Nightstand

    Participants will share a nightstand for common use within the room.

    Shared Dresser

    A dresser will be shared by participants for storing clothing and personal belongings.

    Shared Closet Space

    Closet space is shared, and participants are encouraged to keep their belongings organized.

    Individual Locked Storage

    Each participant has individual locked storage to secure personal valuables and medications. Participants must maintain their own lock and key for this storage.

    Private Rooms

    Participants in private rooms are provided with:

    • An individual nightstand, dresser, and closet space.
    • Individual locked storage for securing personal items and medications.
    • The same bed and bedding as described above.
    • Bathroom Access

    All participants will have access to a shared bathroom, which includes a faucet, mirror, toilet, and shower.

    Participants are responsible for keeping the bathroom clean and reporting any maintenance needs to staff immediately.

  • Code of Conduct

  • Code of Conduct

    • No using of any Narcotics, Opiates or Alcohol on or off the premise.
    • No alcohol/drug use is acceptable while in Sober Living. Use will result in discharge.
    • No abuse of any kind to staff or other residents including but not limited to; Physical, Emotional, Sexual, and/or Verbal.
    • All residents will abstain from all mood-altering substances, unless prescribed by a physician and used as prescribed.
    • All residents will notify the House Manager immediately of any prescribed or over the counter medication that they are taking for any medical condition.
    • You cannot give your medication to other residents.
    • Any suspicion of usage of Alcohol or Drugs will initiate an immediate test for said Alcohol/Drug usage.
    • All residents agree to random periodic drug and alcohol tests. Failure to comply with either will be taken as admittance to drug or alcohol use and will result in immediate eviction for misconduct.
    • Attendance at house meetings is required as scheduled, as well as various household activities.
    • No tobacco use inside, or outside the front part of the premises. Tobacco use is allowed in a designated area in the back yard.
    • No vapor and/or electronic cigarettes are to be used inside, or outside the front part of the premises. Vapor and electronic cigarettes are allowed in a designated area in the back yard.
    • Parking will be limited and only allotted to the residents involved in the Plugged In Recovery program. All
    • Parking shall be on-site and comply with LDC Article 4.2: Off-Street Parking and Loading Regulations.
    • No pets allowed.
    • No gambling allowed within the premises of the home.
    • Appropriate clothing must be always worn.
    • Shirt and pants (or shorts) are required at all times in common areas of the home.
    • All residents agree to help keep the house clean.
    • All residents agree to perform and carry out a designated daily chore based on a chore list that is posted weekly by the house manager.
    • All residents agree to keep their rooms and bathrooms clean and tidy daily.
    • House Managers are granted privileged access to any resident’s room at any time, with or without the consent of the resident.
    • Rules of common courtesy, respect, cleanliness, and cooperation are in order in all common areas.
    • Personal music and other personal devices must be kept at a low level to not disturb other residents.
    • All residents agree not to lend or borrow any money from anyone in the house or sell any property to any resident at any time.
    • Residents will need to inform the House Manager 24 hours before they need to stay overnight at another property.
    • Residents are allowed 2 overnights per week. When the resident returns a test for alcohol and/or drugs will be administered at the discretion of the House Manager.
    • No loitering is allowed within a five-block radius of the house.
    • Residents will need to give Plugged In Recovery LLC a 1-week notice upon the intent to leave the program.
    • All residents agree that if they currently are, or become involved in a relationship while a resident, that it will not interfere with their recovery.
    • Sexual relationships among residents and/or staff is not allowed while currently a resident of Plugged In Recovery LLC.
    • Residents must attend 4 12 Step Meetings unless hindered by work or school and/or approved by the House Manager.
    • Residents must be respectful of peers, staff, and themselves.
    • Residents must honor the confidentiality and privacy of housemates.
    • Residents must make their beds every day and keep their room clean, neat, and orderly.
    • Bathrooms must be clean and spotless at all times. Towels and personal belongings are to be put away after each use and not left out on countertops or floors.
    • All Residents are required to participate in a house meeting and deep clean of the house once a month.
    • No food or drinks are allowed in bedrooms.
    • Water only in bedrooms!
    • No dishes are to be left out on countertops or in the sink! Dishes must be immediately washed and put in the dish rack to dry or rinsed and put in the dishwasher.
    • All food must be eaten in the dining room or kitchen area. Personal food and drink items are to be kept in designated cabinets with a name on them.
    • Noise levels in house and backyard must be kept to minimum during the hours of 9pm-8am
    • Sponsors are the only approved visitors at house at anytime
    • Rent must be paid every Friday by 10:00pm to Wade or the House Manager.

    Participant Complaint Rights

    Participants have the right to file a complaint regarding the sober living home, manager, or licensee. Complaints should be submitted through the Plugged In Portal and will be handled in accordance with the Plugged In Recovery’s established complaint process. Participants can request information about the process from staff and are assured that all complaints will be reviewed and addressed promptly and fairly.

    Termination of Residency

    Immediate Termination Without Notice

    • Residency may be terminated immediately without notice if the participant's behavior poses an immediate threat to the health or safety of themselves or others in the sober living home.

    Termination With Seven-Calendar-Day Written Notice

    • Submitted on December 8, 2025, 8:41 PMResidency may be terminated with seven-calendar-day written notice for nonpayment of fees, charges, or deposits, or for violations specified in subsection (B)(3).

    Termination With 14-Calendar-Day Written Notice

    • Residency may be terminated with 14-calendar-day written notice for any reason not covered under immediate termination or seven-day notice.

    Public Assistance Benefits

    Plugged In Recovery LLC may not require a participant to relinquish their public assistance benefits, including medical assistance, cash assistance, or supplemental nutrition assistance program (SNAP) benefits, as a condition of participation in the program.

    Commitments

    All residence of Plugged In Recovery LLC are required to have a 40 hour per week commitment that can include one or any combination of the following:

    • Employment
    • I.O.P./ Continuing Care
    • Enrolled Student
    • 12-Step Service Commitment
    • Volunteer/ Community Service
    • Pre-approved facility projects

  • Medication Policy

  • Medication Policy

    At Plugged In Recovery, resident safety and recovery stability are priorities. To support a structured and substance-free environment, the following Medication Policy applies to all residents:

    Approved Prescribed Medications Only
    Residents may only possess and take medications that are currently prescribed to them by a licensed medical provider. Medications must be taken exactly as prescribed. Failure to take medications as prescribed is considered substance abuse and may result in corrective action or discharge.

    Controlled Substances
    Controlled substances that are not considered Medication Assisted Treatment (MAT) require prior approval from Plugged In Recovery management before the resident may bring them into the home. Approval is granted on a case-by-case basis and may include additional monitoring requirements.

    Medication Assisted Treatment (MAT)
    MAT medications are approved without restriction as long as the medication is taken as prescribed and the resident maintains a valid and current prescription. Residents must also comply with all storage and reporting requirements in this policy.

    Medication Storage Requirements
    All medications must be kept in their original prescription bottles with the resident’s name, medication name, dosage, and pharmacy label intact. Medications may not be shared, sold, or given to any other resident. Locked medication storage is available upon request.

    Reporting and Documentation
    Residents must notify the House Manager immediately of any new prescription, any change in dosage, or any update or discontinuation of existing medications. Residents agree to participate in routine or as-needed medication reviews for safety and compliance.

    Accountability
    Residents are responsible for safeguarding their own medications and ensuring compliance with this policy. Violations of this policy—including misuse, diversion, unauthorized possession, or failure to report changes—may result in disciplinary action, up to and including discharge from the program.

     

  • Relapse Policy

  • Plugged in Recovery LLC has a zero-tolerance policy.

    In the event of a relapse, you (the participant) will be required to leave the house and stay sober for a minimum of 24-72 hours at owners’ discretion.

    After the 24–72-hour period has passed you can be re-assessed for entry back into the house, and the Plugged In Recovery program by the House Managers and Owners of Plugged In Recovery LLC.

    If a re-occuring relapses occur, you (the participant) may be required to leave the house and the Plugged In Recovery for a minimum of 30 days.

    Failure to comply with the terms of the Relapse Policy and/or re-assessment can result in immediate Eviction for Misconduct from the Plugged in Recovery Program and/or Termination of the Plugged In Recovery Contract.

    Medical Clearance

    All participants entering or reentering the sober living program are required to provide proof of sobriety. This includes submitting a negative urine analysis (UA) sample at the time of admission. If a participant is unable to provide a negative UA sample, they must submit documentation from an approved medical facility confirming their eligibility for the program. This documentation must demonstrate that they are in active recovery and medically cleared to participate in a sober living environment.

    Failure to comply with this requirement may delay or prevent admission into the program. Participants are encouraged to address any concerns or circumstances with staff prior to entry or reentry.

  • Discharge Policy

    Plugged In Recovery is committed to maintaining a safe, supportive, and accountable environment. To ensure the well-being of all residents and staff, the following Discharge Policy applies:

    Immediate Discharge Without Notice
    Residents will be immediately discharged without notice if they pose a threat to themselves or others. This includes any behavior that endangers personal safety, public safety, or the well-being of the community.

    Immediate Discharge With Notice
    Residents may receive an immediate discharge with notice for violations of the Resident Agreement, Code of Conduct, or any behavioral issues that conflict with program expectations. Notice may include a behavior contract, relapse form, or any other documented infraction indicating the resident is no longer eligible to remain in the program.

    Notification of Emergency Contact
    When a resident is discharged, staff will make an attempt to notify the resident’s listed emergency contact. Notification attempts will be documented in the resident’s file.

    Discharge Documentation
    All discharges—whether immediate or planned—will be fully documented. Discharge paperwork and related records will be stored in the resident’s file for compliance, internal review, and quality assurance.

    All residents are required to provide and maintain at least one emergency contact on file at all times. This information must be accurate, current, and updated promptly if changes occur.

    24 Hour Notification

    Emergency contacts will be notified within 24 hours in the event of a situation in which a resident suffers loss of life, experiences a medical emergency, or is involved in an incident that requires EMS to be contacted.

    72 Hour Notification

    Emergency contacts will also be notified within 72 hours of a resident’s discharge from the program. All notification attempts will be documented in the resident’s file.

    Plugged In Recovery is a CLIA-waived program committed to maintaining a safe, supportive, and drug-free environment for all participants. In accordance with CLIA standards and state compliance requirements, we utilize 12-Panel Urine Drug Test Cup (CLIA-waived) to conduct regular drug screenings.

    This policy outlines procedures for test administration, documentation, and the handling of results to ensure accountability and consistency in our sober living homes.

    Policy Statement

    1. All residents may be subject to drug testing throughout their stay at Plugged In Recovery.

    2. Testing will be conducted randomly, routinely, or based on reasonable suspicion of substance use.

    3. Test results are confidential and will be used solely to support recovery and maintain a safe sober living environment.

    4. All test results will be documented and stored in compliance with CLIA guidelines and program policy.

    12-Panel Drug Test

    1. Heroin Metabolite (HMO): 1000 ng/mL

    2. Cocaine (COC): 100 ng/mL

    3. Amphetamine (AMP): 500 ng/mL

    4. Methamphetamine (MET): 500 ng/mL

    5. Marijuana (THC): 50 ng/mL

    6. EDDP (Methadone Metabolite): 100 ng/mL

    7. Methadone (MTD): 300 ng/mL

    8. Morphine (MOP): 300 ng/mL

    9. Oxycodone (OXY): 100 ng/mL

    10. Benzodiazepines (BZO): 300 ng/mL

    11. Fentanyl (FYL): 10 ng/mL

    12. Buprenorphine (BUP): 10 ng/mL

    Alcohol Test

    Plugged In Recovery uses breathalyzer devices to regularly monitor and ensure residents remain alcohol-free, supporting a safe and sober living environment.

  • Swimming Pool Policy

  • PLEASE READ CAREFULLY AND SIGN BELOW

    This Agreement waives any and all liability of Plugged In Recovery LLC for use of the facility, including but not limited to residency, services, and property.

    I, signing this agreement, wish for myself and my child/children to utilize the services, facilities, and swimming pool/whirlpool offered by Plugged In Recovery LLC. I hereby agree that the use of the services, facilities, and swimming pool is voluntary and at my own risk. As a condition of my use of such services, facilities, and swimming pool, I, on behalf of myself, expressly agree to forever discharge, waive and release Plugged In Recovery LLC. Its owners, management, insurers, and employees from any and all claims, demands, injuries, damages, actions, or courses of action, for all acts of active and passive negligence on the part of the company on account of physical injury, mental anguish, death and/or property damage arising from any accident, loss, damages or injury suffered by myself or others resulting from, connected with or caused by my/our use of the Plugged in Recovery LLC services, facilities, and swimming pool, including, but not limited to any injury resulting from mechanical defects or failure of any equipment or devices used in such services, facilities, and swimming pool.

    I agree and, by signing this Agreement, confirm that I am aware that residing at Plugged In Recovery LLC and using the swimming pool and other facilities is done so at my own risk. I understand that there is no lifeguard on duty at the swimming pool. I understand no pets are allowed in the pool area. I understand there is no running or horseplay in the pool area. I understand there is no diving. I understand there is no food or drink in the pool area. I understand there are no glass containers or alcohol, or drugs allowed in the pool area.

  • Liability Release

  • This document is a waiver of your legal rights.

    Please read before signing.

    The undersigned client has requested Plugged In Recovery LLC, and all its volunteers, sponsors, officers, directors, agents, and other representatives to allow client to participate in the Plugged In Recovery programs or reside in the home operated by Plugged In Recovery LLC. In consideration for Plugged In Recovery LLC and affiliated individuals allowing client such privileges, the undersigned client hereby agrees to bind himself/herself, his/her heirs, successors, and his estate to the conditions described in this liability release. The Client understands that the participation in Plugged In Recovery LLC programs and or living in a Plugged In Recovery LLC home may involve risk of injury, harm, danger to personal property, or death to the client; and agrees that such risk shall be fully assumed by the client. In addition, and to consideration of Plugged In Recovery LLC and affiliated individuals allowing client to participate in Plugged In Recovery LLC programs, or reside in a house operated by Plugged In Recovery LLC; the client hereby releases and agrees to hold Plugged In Recovery LLC and affiliated individuals harmless for, and from and against all liability, damagers and claims of any kind, known and unknown, which may be connected with, result from, or arise out of the participation in the Plugged In Recovery LLC programs including without limitation, any and all claims resulting from transportation. I take full responsibility for, release and hold harmless, Plugged in Recovery Sober Living and their owners and agents from any and all liability, claims, or causes of action that I may hereafter have for injuries or damages arising out of my residency at Plugged In Recovery Sober Living. I agree that I will not sue or make a claim against Plugged In Recovery LLC for damages or other losses sustained as a result of any injury, or death, sustained from my residency at Plugged In Recovery LLC I agree that I will not hold Plugged In Recovery LLC responsible for any lost or stolen property. I hereby recognize that my behavior and conduct in Plugged in Recovery LLC could potentially lead to intentional or unintentional bodily injury or injury to property and will not be tolerated. I hereby recognize that this Release of Liability, Waiver of Legal Rights, and Assumption of Risk is a contract pursuant to which I have released any and all claims against Plugged In Recovery LLC resulting from any injury, or death, sustained from my residency including any claims for negligence of the Released Parties. Participant understands, agrees, and promises as follows: Participant shall not take any action or fail to take any action which would cause harm to participant (I.E., self-mutilation, drug or alcohol overdose, suicide attempts through any means) or to others. Participant agrees that any minor children are the participant’s responsibility and that the foregoing release extends to all such minor children. Plugged In Recovery LLC is a clean and sober environment Drugs, alcohol, mind-altering and mood-altering substances are strictly prohibited from use or possession by the participant. Prescription drugs and over-the-counter medicine shall be self-administered. Any violation of the foregoing or any violation of any rule, or instruction from Plugged In Recovery LLC or any affiliated individuals, may result in participant being required to leave Plugged In Recovery LLC’s premises. The appropriate response to a violation of this paragraph or of any rule or instruction shall be determined by Plugged In Recovery LLC sole and absolute direction in a case-by-case basis.
