SOME OF THE QUESTIONS BELOW REQUIRE A RESPONSE FROM THE OTHER PARTY IN THE TRANSACTION. PLEASE FORWARD THE LINK TO THIS FORM TO THE OTHER PARTY SO THEY CAN FILL OUT THEIR PORTION.

If you are the buyer, and the seller has already had their portion completed/notarized, and you are just bringing that paperwork in to register, the seller does not have to complete this form.

ALL PARTIES: Please submit this form at least 2 HOURS PRIOR TO YOUR APPOINTMENT (or by 2:00pm the day before your appointment, if you have an early morning appointment) so we can best serve you.

PLEASE BE SURE TO BRING PHYSICAL COPIES OF ALL DOCS THAT YOU UPLOAD ONTO THIS FORM WHEN YOU COME INTO OUR OFFICE. The copies you upload here are just to help us get your file started and save you time, but these often don't print well, and we must have clear copies for our file.

This form contains a section for the SELLER to fill out, and a section for the BUYER to fill out. BOTH SELLER & BUYER, please also see the VEHICLE INFORMATION section below and fill out what you can within that section.

Louisiana law requires insurance on a motorized vehicle before a license plate can be issued. Please have your insurance in place prior to your appointment at No Delay.

Does the current owner(s) have a current, clear title in their name? If not, what is the situation (is it lost, still financed, etc)?

BE SURE NOT TO SIGN THE TITLE UNTIL YOU ARE IN FRONT OF THE NOTARY! You can bring the title already notarized or we can notarize in our office. Be sure ALL owners on title are present to sign.

Will this be a "Bill of Sale" transaction or an "Act of Donation"?

We can draft and execute both of these documents at our office. If the transaction is an "Act of Donation" and you need our office to draft and execute that document, 2 witnesses will need to be present to complete the documents. We sometimes have a witness or two at our office, but always call ahead before you come to see if you need to bring a witness with you.

If this is an "Act of Donation", what is the relationship between the Donor and the Donee?
