        *You MUST live in the Mansfield ISD school district or your child MUST attend a Mansfield ISD school.

        * If you have more than 6 children (2 years – 12th graders) you will need to fill out this form again for your additional children.

2 – YOU MUST ATTEND ONE OF THE TWO VERIFICATION CHECKS TO BE APPROVED
