You can try a class out for $33. If you choose to continue, we'll apply that to the monthly fee.
Cash Payments and Electronic payments accepted.

Acknowledgment of Risk
I, the undersigned parent or legal guardian, acknowledge that my child is voluntarily participating in the Arts & Crafts Club hosted by Res Novae Academy, (hereafter referred to as “the Host”).

I understand that the program includes art, music, and creative learning activities that may involve the use of materials, tools, instruments, and supplies which could pose certain risks, including minor cuts, allergic reactions, strains, or other injuries. I also acknowledge that while all reasonable safety precautions will be taken by staff and volunteers, the possibility of an unforeseen hazard does exist.

Release of Liability
In consideration of allowing my child to participate, I hereby release, waive, and discharge Res Novae Academy, its administrators, instructors, volunteers, workers, and affiliates from any and all claims, liabilities, or causes of action that may arise from my child’s participation, including but not limited to any injury, illness, loss, or damage to property incurred as a result of participation in the program or use of materials provided.

I further agree not to hold Res Novae Academy or any associated individuals or entities liable for any damages, losses, diseases, or injuries incurred by myself, my invited guests, or any minor listed on this form.

Program Conduct & Supervision
I understand that while the Host will provide supervision during the program hours, I remain ultimately responsible for my child’s behavior and compliance with safety and conduct expectations. I certify that I have discussed with my child the importance of following all safety guidelines and instructions provided by instructors and staff.

I understand that students may be dismissed from the program at any time for behavioral or safety concerns without refund.

Financial Agreement

Tuition: $99 per month per student. Electronic Payments are Accepted for an addtional fee.
Try It Class! We gladly offer the opportunity for families to try our class out to see if it's a match. We do charge $33 per student for this class. We offer this one time per family.
All supplies included.
School Choice/ESA Funds: You will receive an invoice to upload to your student’s account for reimbursement or payment. If your invoice is reject for any reason, you will be required to submit payment either via cash or electronic payment.
All payments must be recieved before a student can attend class, this includes Choose Act Classwallet payments.
All unpaid accounts will be pursued in Jefferson County, AL for payment.
Late fee: $1 for each minute past 7:40. Classes end at 7:30 and we will offer a 10 minute grace period for pick up. All late fees must be paid prior to attending any future classes.

No Refund Policy
By submitting payment, I acknowledge and agree that all fees are non-refundable for any reason, including but not limited to illness, absence, withdrawal, dismissal or schedule changes. Pricing is not based on attendance. No credit or transfer will be issued for missed or cancelled sessions. Class fees are not contingent on attendance.

Property Damage
I understand, acknowledge, and agree that I, on behalf of myself, my child/ward, or invited guests, am solely responsible for any and all damage at full market cost (including repairs, replacements, labor, time or other compensation) to any facilities used such as 4289 Main St, church property, including program property resulting from the actions of myself, my child or guests.

Governing Law
This agreement shall be governed by and construed in accordance with the laws of the State of Alabama, with jurisdiction in Jefferson County, Alabama.

Allergies and Medical Conditions
I understand that it is my responsibility as the parent or guardian to disclose any known allergies, medical conditions, or dietary restrictions that may affect my child’s participation in the program.

I acknowledge that Res Novae Academy cannot guarantee that the facility or program activities are free from common allergens, including but not limited to peanuts, tree nuts, dairy, gluten, soy, eggs, or other potential food and environmental allergens.

The Host will make reasonable efforts to promote a safe environment but cannot ensure an allergen-free space or prevent exposure through other participants, materials, or shared surfaces.

Parents and guardians are encouraged to provide any necessary allergy plans or emergency medication (e.g., EpiPens) for their child and to ensure the child understands their own safety precautions.

Medication Policy
I understand and agree that Res Novae Academy staff, volunteers, or representatives are not permitted to administer any medications, including prescription or over-the-counter drugs, to students at any time.
If my child requires medication during program hours, it is my sole responsibility as the parent or guardian to administer the medication directly or to make appropriate arrangements to do so.
In the event of a medical emergency, staff will call 911 and notify the parent or emergency contact listed on the registration form.

Acceptance of Terms
By signing below, I certify that I have read this document and fully understand its contents. I am aware that this is a release of liability and a contract, and I sign it of my own free will.

I,   First Name*   Last Name*   , understand and acknowledge that students arriving more than 15 past the start time will not be admitted for that class. Doors are locked at 6:45. Doors will open at 7:15 for student pick up.
