1. Assumption of Risk
I understand that martial arts involves physical activity, contact, and risk of injury, including but not limited
to sprains, bruises, broken bones, or more serious injuries. I voluntarily choose to participate and assume
all risks.
2. Release of Liability
I release and hold harmless Absolute Martial Arts, its owners, instructors, staff, and agents from any and
all claims, demands, or causes of action arising out of my participation, including injury, illness, property
damage, or death, whether caused by negligence or otherwise.
3. Medical Fitness
I certify that I am physically able to participate in martial arts activities and have disclosed any health
conditions to the staff. I authorize emergency medical treatment if necessary.
4. Media Consent
I allow use of photos and video of myself / my child for school promotion and online media content.
5. Parental Consent (if under 18)
As parent/legal guardian, I consent to the participation of my child/children in martial arts activities and
agree to the terms of this waiver on their behalf.
6. Acknowledgment
I have read and understood this waiver. I voluntarily sign it and understand that by doing so I give up
certain legal rights.
