CHF has prepared this form to streamline the Health Clinic at the 2026 Collie Club of America National Specialty. Signing up means that all forms for your dogs' samples will already be filled out before the national. Dogs signed up using this system will be given priority at the CHF Health Clinics.

Eye Checks & Coloboma Study:
This study needs samples from dogs with choroidal hypoplasia, as well as from dogs with choroidal hypoplasia and colobomas or detachments.
- Current and any/all Previous Ophthalmology Eye Exams (note: for CEA mild dogs, an adult eye check is required)
- CEA Genetic test Result
- 5-6 Generation Pedigree
- Any other known eye conditions

- What illnesses or diseases your dog has experienced
Epilepsy Study, if affected:
This study is looking for both affected dogs and unaffected dogs over 8 years old with no epileptic relatives.

- Relatives with epilepsy
If affected:
- Age of Onset
- Name of Diagnosing Veterinarian
- Known test results
- Seizure description
- Total number of seizures
- Frequency of seizures
- Hospitalization
- Any other conditions or injuries
Please note that free CAER eye checks are only available to dogs participating in the Coloboma study. For all other dogs, the cost of the CAER eye check will be $55.
