This form is for anyone that wants to rent our building for an event. Please fill it out with as much detail as you can. None of the details are set in stone but this will give us an idea on how give you a quote. This form will be submitted to our Pastoral Staff and we will get back to you soon to discuss further details. Unless you have already been pre-approved to use the building by one of the Pastors.

Alcohol, drugs, vaping, and smoking are prohibited on the property and you will let the attendees know that. I agree to use Sandy Creek United Wesleyan Church Facilities as described on this form. I, and the people I am representing agree to leave the Facility in as good or better condition than we found it according to the principles of good stewardship.
I accept that the church is not responsible for injuries that may occur during my event
I acknowledge and agree that Sandy Creek United Wesleyan Church, including its pastors, employees, and volunteers, shall not be held liable for any injury, loss, or damage incurred by myself or any participants during the use of church facilities. I further understand and agree that the church is not responsible for any lost, stolen, or damaged personal property.I accept full financial responsibility for any damage to church property caused by myself or any members of my group during the event and agree to promptly report any such damage or breakage to a pastor or designated church representative.
