We're happy to have you attend our workshop! Register and pay for your ticket(s) using this form.  All attendees need to pay an admission fee. This form is to be filled out by those aged 18 and over. If you are under age 18, please have a parent or guardian complete and submit this form for you.

Please note: a parent or guardian of girls ages 17 and under must attend the workshop for the entirety of the event. Parents/Guardians will not receive supplies. 

Refund Policy All workshop registrations are non-refundable, but may be transferred to another attendee with prior notice. 
Photography/Videography Policy Photos and videos may be taken during the workshop and used for Christian Miss promotional purposes. By attending, you grant permission for your child to appear in event media.

Behavior Expectations  We ask all participants to show kindness, respect, and cooperation throughout the workshop. Christian Miss is a safe, uplifting environment where every girl is encouraged to shine.

Participant Consent and Release of Liability

I hereby acknowledge and consent to my or my child's participation in Christian Miss workshops and all related events, activities, and programs (collectively referred to as “Christian Miss Events”).

I understand and accept that participation may involve physical activity and other risks inherent to such events. I voluntarily assume all risks and responsibilities associated with my participation and agree to release, indemnify, and hold harmless Christian Miss Organization, Inc., Christian Miss LLC, and all of their respective officers, directors, agents, representatives, employees, volunteers, affiliates, successors, and assigns (collectively, “Christian Miss Entities”) from any and all claims, liabilities, demands, or causes of action of any kind that may arise from or relate to my participation in Christian Miss Events.

I further agree to waive any and all claims against Christian Miss Organization, Inc., Christian Miss LLC, and the Christian Miss Entities, including but not limited to their staff, affiliates, participants, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the events.

I understand that my or my child's participation is voluntary, that I am responsible for my or my child's well-being during all Christian Miss Events, and that this release is intended to be as broad and inclusive as permitted by law.
