Our staff are using this form to collect information for those interested in scheduling a dental appointment at Scenic Bluffs' Reedsburg Dental Clinic, opening January 2026. Patients will be put on a waitlist and be contacted to schedule an appointment when scheduling opens.

Please note that once you establish care with a provider at one of our clinics, that provider will remain your primary provider for ongoing care. This approach supports continuity of care, ensures familiarity with your health history, and helps us deliver the best possible care experience. As a result, continuing treatment may require visits at the same clinic location where you initially established care.
