Volunteer Confidentiality Statement

This statement represents my understanding and agreement to keep all information obtained through A41, Inc. (DBA All for One Ministries & The Branch Gathering Place + Coffee House) in strict confidence. Information may include, but is not limited to: data, materials, knowledge, and communication generated through or originating from A41, Inc., or from any individuals associated with our ministries, including participants, volunteers, staff, and community partners. All such information is considered privileged and confidential and may not be disclosed to any third party.

All pages, curriculum, forms, documents, printed materials, policies and procedures, conversations, messages, resources, contacts, digital lists, internal communications, and any information relating to participants, volunteers, staff, operations, or programs are confidential and the sole property of A41, Inc., regardless of format. This includes hardcopy, photocopy, electronic files, digital media, or any other form of recordkeeping or communication.

Participant information, including any details contained in files or electronic systems, may not be shared with any outside individual or entity under any circumstances without prior authorization from the supervising A41 staff member and the Executive Director.

Any disclosure, misuse, copying, or transmission of confidential information-whether intentional or unintentional-may result in disciplinary action, removal from volunteer service, and/or legal action in accordance with A41, Inc. policies and applicable laws.

Iunderstand that A41, Inc. (DBA All for One Ministries & The Branch Gathering Place + Coffee House) does not provide insurance coverage for my personal vehicle in the event of an accident or damage while volunteering.

I understand that A41, Inc. does not provide insurance coverage for my personal belongings in the event of loss, theft, damage, or accident while volunteering.

I understand that A41, Inc. does not provide worker's compensation coverage for injuries that occur while volunteering or resulting from volunteer activities.

I consent to a background check to be conducted by A41's chosen vendor, should I be asked to volunteer.

Iaffirm that I will adhere to all policies and guidelines set forth by the Board of Directors of All for One Ministries throughout the duration of my volunteerism with the organization.

I affirm that I am of lawful age and legally competent to sign this release. I acknowledge that the terms herein are contractual and voluntarily agreed upon. I further acknowledge that I have fully informed myself of the contents by reading this document before signing it.

I hereby grant permission for A41, Inc. to photograph, video record, or otherwise capture my likeness for ministry-related purposes including, but not limited to: promotional materials, fundraising, press releases, publications, digital platforms, and social media. I understand that I will not receive compensation for the use of such images and waive any future rights to compensation or rights of ownership. All images and media captured remain the property of A41, Inc.

My signature signifies that I agree to these terms and will abide by, adhere to, and honor all statements above. I also certify that my electronic signature provided here and in any future documents is the legal equivalent of my manual signature.
