  • Fill out this form to respond to an ethical complaint.

    Before you start, gather all the information you will need. This includes a clear explanation of what happened from your point of view, any documents you plan to upload, and signed releases if you are sharing confidential information. If you do not have signed releases, be sure to remove or block out any confidential details before submitting.

    Your response is due within 20 days from the date you received this notice. If you do not respond, the Board will assume the complaint is valid and will proceed based on the information available. If you do respond, your statement will be shared with the person who filed the complaint so they can review it and provide a rebuttal. After that, the Board will review all information and decide whether to begin an investigation.
