Important Inspection Disclaimer
This intake form is used to assist in the preliminary review and scheduling of chimney and venting system services. Information and photographs provided are used to better understand your system, identify potential concerns, and determine service needs, job scope, and required equipment.
This intake review is not a formal inspection and does not constitute an NFPA 211 Level I, Level II, or Level III inspection. System condition, safety, and code compliance cannot be confirmed without an onsite inspection performed in accordance with NFPA 211.
Limitations of Visual Information
Photographs, videos, and customer-provided descriptions may not accurately represent actual conditions due to angle, lighting, resolution, obstruction, or distortion. Critical components—including concealed flue interiors, clearances to combustibles, structural enclosures, and attic or chase areas—cannot be evaluated through images alone.
Access Limitations
Only visible and disclosed areas can be considered during this preliminary review. Areas that are concealed, enclosed, sealed, or otherwise inaccessible cannot be evaluated. NFPA 211 requires inspection of accessible areas appropriate to the inspection level, which cannot be fulfilled through this intake process.
