  • Important Inspection Disclaimer

    This intake form is intended to assist in the preliminary review and scheduling of onsite chimney and venting system services. Information and photographs submitted by the customer are used only to help determine service needs, job scope, safety considerations, and whether additional personnel, equipment, or referrals may be required.

    This intake review is not a formal inspection and does not constitute an NFPA 211–compliant Level I, Level II, or Level III inspection.
    NFPA 211 defines the standards for the inspection, evaluation, and maintenance of chimneys, fireplaces, venting systems, and associated solid-fuel–burning appliances. Findings cannot be confirmed or certified without an onsite inspection performed in accordance with NFPA 211.

    Limitations of Visual Information
    Photographs, videos, and customer-provided descriptions may not accurately depict existing conditions due to angle, resolution, lighting, obstruction, or camera distortion. Many critical components of a chimney or venting system—including concealed flue interiors, clearances to combustibles, structural enclosures, and attic or chase areas—cannot be evaluated through images alone.

    Access Limitations
    The customer understands that the inspector may not be able to access or view all portions of the chimney or appliance system during a preliminary review. NFPA 211 requires that inspections include all “readily accessible” and “accessible” areas applicable to the inspection level. Any areas that are concealed, sealed, enclosed, or physically inaccessible cannot be evaluated through this form.

    No Warranty or Certification
    No representations, warranties, or safety certifications are made based on this intake form or the information provided herein. A complete assessment of system condition, safety, compliance, construction, draft performance, and operational integrity can only be established after an onsite NFPA 211 inspection.

    Inspector Liability Statement
    The inspector is not responsible for identifying deficiencies, unsafe conditions, improper installation, or code violations that are not visible, not disclosed, or not accessible through customer-submitted materials. Any preliminary comments or suggestions made before the onsite visit are advisory only and shall not be considered a formal inspection or professional determination of system safety.
