I acknowledge that Struttura cannot control or be responsible for other products, treatments, medications, or behaviors I may be using concurrently.

I confirm that I have fully and accurately disclosed all relevant information, including but not limited to:

 

Current or recent medications
Accutane (current or within the last 12 months)
Birth control or hormonal treatments
Pregnancy or breastfeeding
Skin conditions (eczema, rosacea, dermatitis, etc.)
Allergies or sensitivities
Recent peels, lasers, microneedling, or cosmetic procedures
