Membership Cancellation Agreement Terms By submitting this form, you acknowledge and agree to the following terms regarding your membership cancellation at The Office by BR The Network: - Membership Commitment: Membership is subject to the terms and duration as agreed upon at the time of sign-up. - Cancellation Request Procedure: To request cancellation, you must complete and submit this agreement in full. Incomplete forms may delay processing. - 90-Day Notice Requirement: A minimum of 90 days' written notice prior to the intended cancellation date is required. Failure to provide adequate notice may result in continued billing and/or forfeiture of deposits. - Refund and Early Termination Policies: Refunds of any prepaid amounts or deposits are subject to review and will be processed according to the original membership agreement. Early termination may incur additional fees as specified in your contract. - Reactivation Terms: Should you wish to reactivate your membership after cancellation, a new agreement and applicable fees may apply. Reactivation is subject to availability. - Exceptions: Any exceptions to these terms must be approved in writing by management. - Nonpayment Conditions: Nonpayment of dues or outstanding balances may result in immediate suspension or termination of membership, regardless of notice provided.
