RESPONSIBILITIES
I understand that MPB/EVB Managers (Head Coaches), Assistant Coaches, Team Parents, and Board Members hold responsibilities both on and off the field and are expected to demonstrate exemplary citizenship and sportsmanship in accordance with PONY Rules, League By-Laws, Official Baseball Rules, and league guidelines. Failure to comply may result in removal from my volunteer position at the discretion of the MPB/EVB Board of Directors.
BACKGROUND CHECK
I acknowledge that all volunteers are subject to a background check and must submit a completed volunteer application with a valid, current driver’s license. Managers, Assistant Coaches, and Board Members must also complete required Concussion Course and Child Abuse Mandated Reporter training, in addition to passing a Live Scan. No adult may participate in any league activity without meeting these requirements. I consent to all background investigations and understand that approval and continued volunteer status are at the sole discretion of MPB/EVB.
MEDICAL
I understand that participation involves inherent risks, including exposure to infectious diseases, and I knowingly assume all such risks, whether known or unknown, to the fullest extent permitted by law.
LEGAL
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (Menifee Pony, Inc., Elsinore Valley Baseball, Pony Baseball/Softball Inc.) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
