Thank you for nominating outstanding older adults in your community! This form is for Area Agencies on Aging (AAAs), regional Long Term Care Ombudsman Programs, and HIICAP organizations to nominate up to two individuals for NYSOFA's Older New Yorkers' Day recognition event, which is being celebrated in person on May 26, 2026 (invitation to follow) in Albany.
Please complete this entire form so that we have information on all nominees for our outreach efforts and to include bios in the Older New Yorkers' Day program book as well as in NYSOFA Director Greg Olsen's opening remarks for the ceremony.
GENERAL INSTRUCTIONS
- Please complete a unique form for each of your nominees. This form captures biographical information as well as photos.
- For consistency, we are asking nominees to follow the specified word limits. NYSOFA's Communications Team will edit all bios for length and clarity.
- Please also follow the deadlines below to assure that your nominee is included in the ceremony and program book that NYSOFA is preparing in advance of Older New Yorkers' Day.
- Nominators should work to ensure the accuracy of all information, especially the spelling of names, as NYSOFA will be making all editorial decisions on the basis of the original submissions and can’t independently verify all nominee information.
- Nominees will also be considered for special award consideration: One nominee will receive the Governor's Exemplary Service Award, up to two nominees will be selected as Long Term Care Ombudsmen of the Year, and up to two nominees will be selected as HIICAP Counselors of the Year.
NOMINATION DEADLINES
All materials, including nomination bios and photos, are due March 13, 2026.
ADDITIONAL NOTES
NYSOFA will be sharing nominees' contact information with a select list of individuals, including legislative offices and the media (on request). If your nominee would not like to have their contact information shared, please click the opt-out button in the form.
All submissions are restricted to AAAs, LTCOP/HIICAP programs (for the special awards outlined above) and/or their designees for submission. NYSOFA will consult with the AAA, LTCOP/HIICAP for any final decisions or questions about the status or standing of submissions for consideration, and any other submissions will not be considered.
Our form provides a way to save your work by creating a unique link to pick up where you left off. However, we highly encourage nominators to collect their responses in a saved external document, and then cut and paste into the form all in one session to avoid losing any work.
