Who Can Attend:

This trip is open to any youth who are currently in grades 8-12 (2025-26 school year).

Dates/Times:

We will be attending the Main Campus 2 "Worthy" Youth Conference at Franciscan University of Steubenville on the weekend of June 26-28, 2026. We will depart from Most Holy Trinity church parking lot at 9:00 AM on Friday 6/26 to travel to Steubenville, OH, and will return to MHT at approximately 7:30pm on Sunday 6/28.

Transportation: Great Lakes Charter Bus

Cost of Attendance:

The cost to attend is $350 or less based on fundraising/sponsorship earnings. This amount covers the conference registration fee, lodging and meals, a group t-shirt, and our transportation costs.

A non-refundable $100.00 deposit is due 6 days within the submission of this form, and the remaining balance is due May 1st, 2026.

Steubenville Online Liability Form: Once the form is released to us from Franciscan University, a link to the form will be emailed to you.

Cost
The cost to attend is 350.00 or less based on sponsorship/fundraising earnings. To secure a spot, a $100.00 non-refundable deposit is required within 6 days of the submission of this form. Payment can be made by check/cash to Most Holy Trinity Parish or online at MHT Online Giving Final balance and payment instructions will be emailed by April 1st. Final payment is due May 1st.

Parent/Guardian Permission

I hereby consent to participation by my son/daughter named on this form to attend the Steubenville "Worthy" Youth Conference at Franciscan University, Steubenville, Ohio from June 26 June 28, 2026. I understand that this event will take place away from the parish/school grounds and that my son/daughter will be under the supervision of the authorized parish/school personnel/ volunteer chaperones. In consideration of my son/daughter being allowed to participate in this event, I agree to waive and release, and indemnify and hold harmless Most Holy Trinity Parish/School, any and all affiliated organizations, its/their employees, agents, representatives, volunteers and drivers, from any and all claims I or my son/daughter may have, excluding claims for intentional misconduct or gross negligence, arising from or relating to my son/daughter participation in this event. I consent to the stated conditions for participation in this event, including the method of transportation. It is also understood that I am responsible for transporting my child upon his/her return to MHT church parking lot on Sunday, June 28.
