WAIVER AND RELEASE OF LIABILITY FORM

NOTICE OF CONDITIONS AND ASSUMPTION OF RISK
I acknowledge that the notary services I am receiving are conducted at a private, residential property. I understand that access to the property, including the driveway, parking areas, walkways, and surrounding grounds, may be affected by weather-related conditions, including but not limited to snow, ice, freezing temperatures, sloped surfaces, uneven ground, or limited traction.

I voluntarily choose to enter the property and assume all risks associated with driving, parking, standing, or walking on the premises, including the risk of my vehicle becoming stuck, sliding, or sustaining damage, and the risk of personal injury.

RELEASE AND LIMITATION OF LIABILITY
To the fullest extent permitted under Michigan law, I agree that the Notary Public and property owner shall not be held liable for any loss, damage, or injury to vehicles, personal property, or persons arising from or related to weather conditions, surface conditions, or terrain while I am on the property.

This acknowledgment does not apply to gross negligence or willful misconduct, which cannot be waived under Michigan law.

ACKNOWLEDGMENT OF REASONABLE CARE
I understand that reasonable efforts may be made to mitigate weather-related conditions; however, I acknowledge that such efforts do not eliminate all risks associated with winter or inclement weather conditions.

VOLUNTARY AGREEMENT
I confirm that:

I have read and understand this document in its entirety.
I have had the opportunity to ask questions.
I am entering the property voluntarily and at my own risk.
I agree to the terms outlined above.

I, the undersigned participant, affirm that I am of the age of 18 years or older, and that I am freely signing this agreement. I certify that I have read this agreement, that I fully understand its content and that this release cannot be modified orally. I am aware that this is a release of liability and a contract and that I am signing it of my own free will.
