I, the undersigned participant and parent or legal guardian (if the participant is under eighteen (18) years of age), hereby acknowledge and agree to the following terms as a condition of participation in the YEG Summer Cup Hockey League:
1. Assumption of Risk
I understand that participation in ice hockey involves inherent risks, including but not limited to bodily injury, concussion, illness, permanent disability, paralysis, or death. These risks may arise from physical contact, ice and facility conditions, equipment, and the actions or inactions of other participants, officials, volunteers, or spectators.
I knowingly and voluntarily assume all such risks, including those arising from negligence, to the fullest extent permitted by law.
2. Release and Waiver of Liability
I hereby release, waive, and forever discharge the League, its directors, organizers, employees, contractors, sponsors, officials, volunteers, and facility owners from any and all claims, demands, actions, damages, losses, or liabilities arising out of or related to my participation in the League, whether caused by negligence or otherwise, to the fullest extent permitted by law.
I agree that I will not bring any claim or legal action against any of the above parties in connection with my participation in the League.
3. Insurance Disclosure
I acknowledge and understand that the League maintains general liability insurance only.
I further acknowledge that the League does not provide participant medical, dental, accident, or injury insurance for this division. I understand that I am solely responsible for maintaining my own insurance coverage and for all medical and related expenses incurred as a result of my participation.
4. Medical Authorization
In the event of injury, illness, or medical emergency, I authorize the League and its representatives to obtain emergency medical treatment and transportation on my behalf if deemed necessary.
I understand that I am solely responsible for all medical and related expenses incurred.
5. Indemnification
I agree to indemnify, defend, and hold harmless the League and its directors, organizers, employees, contractors, sponsors, officials, volunteers, and facility owners from any claims, actions, damages, losses, or costs (including legal fees) arising out of or related to my participation in the League, including claims brought by me or by any third party on my behalf.
6. Rules and Conduct
I agree to comply with all League rules, policies, and codes of conduct. I understand that fighting, dangerous play, or failure to follow League rules may result in disciplinary action, suspension, or removal from the League without refund.
7. Photo, Video, and Media Release
I grant permission for the League to use my name, image, likeness, and/or voice in photographs, video recordings, live streams, highlight clips, and other media for promotional, marketing, informational, or social media purposes without compensation and without further approval.
8. Governing Law
This agreement shall be governed by and construed in accordance with the laws of the Province of Alberta.
