I, the undersigned (“Model”), willingly give permission to the Creator/Producer to photograph, film, record, and otherwise capture my image, likeness, voice, and/or performance during the current project as well as all future projects.
I also grant permission for these materials to be used in this project and any future projects created by the Creator/Producer, including but not limited to:
- Marketing and promotional materials
- Social media and online platforms
- Websites, digital media, and printed materials
- Educational, artistic, or commercial projects
This permission is granted worldwide and in perpetuity for any lawful purpose.
Use and Ownership
I understand that all photos, videos, audio recordings, and related materials are the sole property of the Creator/Producer. I waive any right to inspect or approve the final product or its use.
Release of Claims
I release and hold harmless the Creator/Producer and their partners, collaborators, heirs, and assigns from any claims, demands, or liability related to the use of my image, likeness, or voice as described above.
Age & Authority
I confirm that I am at least 18 years of age and legally able to sign this agreement.
If the Model is under 18, a parent or legal guardian must sign below.
