Community Living Support and Respite Provider
(1915(i) HCBS Waiver - North Carolina)
Job Summary:
The Community Living Support (CLS) and Respite Provider delivers individualized, person-centered services to individuals with intellectual and/or developmental disabilities (IDD) who are eligible under the 1915(i) Home and Community-Based Services (HCBS) Waiver in North Carolina. The goal of CLS is to support skill development and independence in home and community settings, while Respite provides short-term relief for primary caregivers.
Essential Duties and Responsibilities:
Community Living Support (CLS):
Assist individuals in developing and maintaining skills related to daily living, communication, socialization, and safety.
Support individuals in community integration and participation activities (e.g., recreational outings, social groups, volunteering).
Provide coaching and support with health and wellness routines, such as preparing meals, personal hygiene, and using assistive devices.
Implement Individual Support Plans (ISPs) and document progress toward identified outcomes.
Use positive behavior support strategies and reinforce skill development in real-world settings.
Respite Services:
Provide temporary care and supervision to the individual in their home or approved community setting.
Ensure the individual's safety, comfort, and emotional well-being while the primary caregiver is unavailable.
Follow established routines and care protocols as directed by the caregiver and ISP.

Respond to emergencies and behavioral concerns using approved de-escalation
and crisis management strategies.
Documentation and Compliance:
Maintain accurate service notes, time logs, and documentation in accordance with
NC Medicaid, 1915(i) waiver, and agency requirements.
Participate in staff meetings, supervision, and required trainings including person-
centered planning, HIPAA, and abuse/neglect prevention.
Ensure services are delivered in the least restrictive, most inclusive setting possible.
Qualifications:
High school diploma or GED required; associate degree or relevant human services
experience preferred.
Minimum of 18 years of age and eligible to work in the U.S.
Valid driver's license, reliable transportation, and acceptable driving record (if
transporting clients).
Ability to pass background checks, drug screening, and required training (e.g., First
Aid/CPR, medication administration if applicable).
Experience working with individuals with IDD or mental health needs strongly
preferred.
Skills and Abilities:
Compassionate, dependable, and patient with strong interpersonal skills.
Able to follow detailed support plans and adapt to changing needs.
Ability to maintain confidentiality and professional boundaries.
Comfortable assisting with mobility, personal care, and adaptive equipment as
needed.
