All individuals must complete this form in its entirety, on behalf of yourself and on behalf of any minors in your care accompanying you, prior to entering our facility or engaging in any activity related to Notorious D.O.G Dog Rescue. This waiver is for the duration of your visit and all future visits and includes, but is not limited to, observing, working with or interacting with any animals, using the facility and equipment in any manner, transporting animals for any purpose, and practicing and/or engaging in other related activities.

RELEASE AND WAIVER OF LIABILITY
This Release and Waiver of Liability (hereby known as “Release”), executed on today’s date by First Name* Last Name* (hereby known as “Volunteer”), in favor of Notorious D.O.G Dog Rescue, an Alabama non-profit, its directors, officers, employees, agents, representatives, successors, and assigns, located at 210 Stokes Street NW, Huntsville, AL 35805 (collectively, “NDR”). Volunteer desires to volunteer in one or more of NDR’s dog rescue activities (collectively, “Activities”). Initials*
Waiver and Release. Volunteer acknowledges that the risks involved in the Activities include, but are not limited to, those caused by the animals, facilities, temperature, weather, condition of participants, equipment, and actions of other people including, but not limited to, participants, volunteers, and monitors. Volunteer does hereby, voluntarily, and without duress execute this Release. Volunteer does hereby release and forever discharge NDR from any and all actions, causes of action, suits, debts, liabilities, claims, and demands of whatever kind or nature, either in law or in equity, which have arisen now or may hereafter arise from Volunteer’s participation in the Activities. Volunteer understands that this Release discharges NDR from any liability or claim that Volunteer may have against NDR with respect to any bodily injury, personal injury, illness, death, property damage, or any other damages that may result from Volunteer’s participation in the Activities. Assumption of Risk. Volunteer acknowledges and agrees that animal bites or other injuries to humans and other animals do occur, and that all animals can carry and transmit diseases, including diseases that can be transferred from animals to humans. Volunteer understands that NDR will not be responsible for any damage an animal may inflict on any person, another animal, or any property, or for the transmission or transfer of any disease or parasite to other animals or people, and no attempt will be made by Volunteer, or anyone on Volunteer’s behalf, to hold NDR responsible for such damages. Volunteer hereby expressly and specifically assumes the risk of injury or harm resulting from Volunteer’s participation in the Activities. Initials*
No Representations. Volunteer acknowledges and agrees that NDR makes no representations with respect to any animals that Volunteer may interact with while participating in the Activities. Initials*
Indemnification. Volunteer agrees to indemnify and hold NDR harmless from any damages, loss, cost, or liability (including reasonable legal fees and the cost of enforcing this indemnity) arising out of or in any way resulting from Volunteer’s participation in the Activities, to include but not limited to, the transportation of animals for any purpose. Insurance. Volunteer understands that NDR does not carry or maintain health, medical, worker’s compensation, automobile, or disability insurance coverage for any volunteer. Volunteer is solely responsible for his/her own care and coverage, should any be desired or needed. Each volunteer is expected and encouraged to make arrangements for appropriate insurance coverage prior to beginning participation in the Activities. Initials*
Fitness to Participate. Volunteer certifies that he/she is mentally and physically fit for any activity in which they intend to participate in at NDR’s facility or elsewhere and have not been advised to not participate by a qualified medical professional. Volunteer certifies that there are no health-related reasons or problems, which preclude his/her participation in this activity. Initials*
Photographic Release. Volunteer agrees that NDR may use images and recordings, whether audio or visual or both, of Volunteer taken during or in connection with the Activities as it sees fit in its sole discretion to benefit and promote NDR and Volunteer does not expect monetary compensation, remuneration, or a fee of any kind in exchange for allowing NDR to use such images and recordings. Volunteer does hereby grant and convey unto NDR all right, title, and interest in any and all photographic images and video and audio recording made by NDR during the Activities, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings and Volunteer acknowledges and agrees that all artistic control is in the hands of NDR. Information Release. I understand the information on this form may be submitted to any party with a legal interest and I release NDR and its officers and employees from any liability whatsoever for supplying such information. Initials*
Termination. Volunteer acknowledges and agrees that Volunteer’s participation in the Activities is a privilege that may be withdrawn at any time by NDR with or without written notice. Initials*
Enforceability. If any provision of this Release is found to be unenforceable in any way, it shall be enforced to the maximum extent possible and all other provisions of this Release shall remain in full force and effect. Initials*
