All individuals must complete this form in its entirety, on behalf of yourself and on behalf of any minors in your care accompanying you, prior to entering our facility or engaging in any activity related to Notorious D.O.G Dog Rescue. This waiver is for the duration of your visit and all future visits and includes, but is not limited to, observing, working with or interacting with any animals, using the facility and equipment in any manner, transporting animals for any purpose, and practicing and/or engaging in other related activities.

If you have already completed a volunteer waiver with us, you do not need to complete another.
