YOUR RESERVATION IS NOT CONFIRMED UNTIL YOU ARE CONTACTED BY A MEMBER OF THE HEALTH MUSEUM'S RESERVATIONS TEAM AND HAVE PAID THE DEPOSIT. PLEASE ALLOW UP TO 5 BUSINESS DAYS TO PROCESS YOUR SUBMISSION. THE INVOICE ATTACHED TO YOUR SUBMISSION ONLY SERVES AS AN ESTIMATE. 

              • Please Note: Field Trips with add-ons require advance reservation of at least 4 weeks. Without a processed deposit, regular general admission prices may apply when you arrive. If full payment is not received within 10 business days of the scheduled field trip, an educator cannot be guaranteed and any add-ons will be cancelled. 
              • Please arrive on time to your reservation, if a class/lab is scheduled, then late arrivals may result in your class/lab being canceled.
              • To be considered a group, a reservation for at least 10 patrons must be made. If your group size changes to less than 10 patrons, you will no longer be considered a group and will be required to pay the general admission rate.
              • If your number of patrons is different upon arrival at the Museum, you will receive vouchers for each absent patron already paid for. NO MONETARY REFUNDS WILL BE GIVEN. Vouchers cannot be applied towards future field trips or group reservations.
              • Groups bringing more patrons than reserved are required to pay for the actual number of attending patrons. Any additional chaperones or patrons will pay the group reservation rate upon arrival.
              • Any “no-shows” or groups that fail to cancel within 10 business days of their scheduled reservation will be billed the FULL reserved amount. Please see our website, www.thehealthmuseum.org, for further policy information.
              • If scheduling multiple reservations, a separate payment is required for each date reserved. If checks include more than one reservation payment, it will NOT be accepted. Please review the reservation for accuracy, and sign and return to us via e-mail within 5 business days. Please call 713-337-8451 or e-mail reservations@thehealthmuseum.org if you have any questions.
