I authorize my child to participate in the trainings and matches organized by Sporting Phnom PenhI acknowledge that I have taken out liability insurance covering my child for Sporting Phnom Penh activities.I understand that the club is not responsible for accidents or damages outside training hours, or when the club is not at fault.I certify that my child is fit to participate in sports activities and accept the normal risks associated with sports practice.I authorize the club staff to take any necessary measures in case of emergency.I accept the club rules and agree that my child will respect them.
