  • You created an account when you applied and do not need a code to register. Please login to your account at https://services.wvlablicense.wv.gov/account/. If you need further assistance with your username or password, please email dholsclp@wv.gov.

  • Perform a search at https://services.wvlablicense.wv.gov/verify/. We recommend searching by first name if your last name has ever changed. 

  • What is the status of your license? If you aren't sure, click the hover link to search.*
  • This form is only used to provide access to individuals who have a WV laboratory practitioner license (MLT, MLS, PA, CT, H, POCT, Trainee). If you need to apply for a license, please visit the LAB-PLUS page and select the applicable option. 

  • You do not need a registration code because your license is not eligible for renewal. Please visit the LAB-PLUS page and select the applicable initial application. You will create your account during the application process.

  • If you received a submission confirmation email then your email address is verified to be valid.

    If it has been less than one week since your first submission, please allow us more time to process the request. 

    If it has been more than one week since your first submission or your situation is urgent, please email us at dholsclp@wv.gov. 

     

  • One-time codes are only for individuals who have not yet registered. If you know your username but don't remember your password, you can request a reset on the login page at https://services.wvlablicense.wv.gov/account/. If you do not remember your username, please email dholsclp@wv.gov with your full name and license number. The password you set when you initially registered overwrote the one-time code and new codes are NOT generated for forgotten usernames and passwords. 

  • PLEASE DO NOT ATTEMPT TO RESET YOUR PASSWORD IF YOU HAVE NOT YET RECEIVED YOUR CODE/CREATED AN ACCOUNT; THIS WILL INVALIDATE THE CODE AND PREVENT YOU FROM REGISTERING.

    Each code, which is 8 characters long, may include letters, numbers, and symbols. All 8 characters must be entered for the code to work. 

    If you are attempting to copy and paste the code, please be sure you are not adding additional characters/spaces or missing characters.

    If you are sure this is not the case, then your browser may not allow you to paste text into a password field; please try typing the code in manually. 

    If you attempted to change your password using the login link, the password you selected replaced the one-time code assigned to you. Return to the registration form at https://services.wvlablicense.wv.gov/account/register/ and enter the password you created.
