I (hereinafter "Owner"), am the legal owner or authorized agent of the dog described above. I hereby authorize South Paw Rescue, its agents, volunteers, representatives, and drivers (hereinafter "Transporters") to transport the above-described dog to and from a scheduled spay/neuter appointment, including any necessary stops related to the appointment.
I acknowledge and agree that I am voluntarily allowing the Transporters to provide this transportation service. I further understand and acknowledge that transporting animals and the performance of veterinary procedures, including but not limited to examinations, vaccinations, anesthesia, spay/neuter surgery, and other related veterinary care, involve inherent risks despite the exercise of reasonable care. Such risks include, but are not limited to, illness, injury, infection, bleeding, swelling, wound complications, delayed healing, adverse reactions to medications or anesthesia, accidental escape, permanent disability, or death.
I understand that post-operative complications may occur even when veterinary procedures are properly performed and appropriate post-operative monitoring and discharge instructions are followed. I acknowledge that South Paw Rescue does not guarantee any particular medical outcome and is not responsible for complications that arise as a result of the veterinary procedure itself or the normal risks associated with recovery.
Owner agrees to carefully follow all post-operative instructions provided by the attending veterinarian. Owner acknowledges that failure to follow such instructions, including but not limited to activity restrictions, medication administration, incision monitoring, use of an e-collar when recommended, or timely veterinary follow-up, may increase the risk of complications. South Paw Rescue shall not be responsible for any injury, illness, or complication resulting from the Owner's failure to comply with post-operative instructions.
South Paw Rescue and its Transporters shall not be held liable for any injury, illness, loss, damage, medical complication, or other harm to the dog arising out of or in connection with transportation, veterinary treatment, surgery, anesthesia, post-operative recovery, or follow-up care, except in cases of gross negligence or intentional misconduct as defined by South Carolina law.
I release, waive, and forever discharge South Paw Rescue, its Transporters, and their respective officers, directors, employees, volunteers, agents, and representatives from any and all claims, demands, causes of action, liabilities, costs, expenses, and attorney's fees ("Claims"), whether known or unknown, arising out of or related to the transportation of the dog, the veterinary procedures performed, anesthesia, surgery, post-operative care, or any complications resulting therefrom, to the fullest extent permitted by South Carolina law.
I further agree to indemnify and hold harmless South Paw Rescue and its Transporters from any Claims made by third parties arising from or related to the transportation, veterinary treatment, surgery, or care of the dog.
By signing below, I acknowledge that I have carefully read and fully understand this Release. I voluntarily agree to its terms, understand that I am giving up certain legal rights, and intend for this Release to be legally binding upon myself, my heirs, successors, assigns, and representatives to the fullest extent permitted by South Carolina law.
