Submission & Media Release Form

Teen Perspectives on Immigration is an art & essay contest hosted by The Immigrant Learning Center (The ILC) for students at Everett High School, Malden High School and Medford High School. Winners will be notified the week of March 23. This form must be filled out by a parent/guardian or by the student if the student is age 18 or older.

Your child's entry may be published in the Teen Perspectives magazine, on The Immigrant Learning Center (The ILC)’s website and excerpted in The ILC’s social media channels. In addition, the entry may be published in appropriate newspapers, journals or other print media. Your name and telephone number may be released to such publications in the event that they wish to contact you and your child in connection with the entry. It is understood that the use of these materials by The ILC will not be for personal gain.

By signing this release, you are agreeing to transfer copyright and other rights for the entry to The ILC, and to release The ILC from any claims relating to the above-stated uses. If applicable, by signing this release, you agree to complete any tax reporting forms necessary.

The Immigrant Learning Center, Inc. (The ILC) is requesting permission for your child’s photo/image and personally identifiable information to be used in
public relations efforts.
