This form is for members who paid for 2026 Challenge, 6 months or 1 year in advance and want to add all-inclusive gym access and would need to fill out monthly.

All-inclusive access includes:
- Weight Room (all floors)
- Cardio Room
- Boxing Room
- Locker Rooms + Showers
- Saunas
- FITCAMP Classes
- Community Classes
- All Benicia Community Health and Fitness Club Classes ( Please see website or front desk for schedule) https://beniciahealthandfitnessclub.com/subpages/classes.html

December 2025 is included at no cost
Month-to-month, no commitment

For Challengers, Starting Feb 1st, if you sign up for monthly membership, gym access will already be included in that fee, you will not need to fill this form out again 2/2026.
