I am not looking for personal information more than just the name(s) and the address on the home, so please cross off any account numbers or other information that is not relevant. I just need to verify that you are the owner of the address.

Renting? I will need a copy of your rental agreement that shows the permission of your pets with the name of your landlord and the address of the property. Or a letter from your landlord indicating that you can have this breed of dog or the number of pets that you have are all allowed on the premises. A personal letter from your landlord must have their contact information included.
