Players must provide a State issued ID, Registration, and Waiver to be eligible to play. 

It is important that everyone understands the goals, boundaries, and expected conducts for players on our team. A "contract" helps us communicate this. Our goal is to help develop young men by providing an environment where they will priactice the values of sportsmanship, teamwork, commitment, and hardwork. Therefore, the following guidelines are how we will act as players. 

- I will listen to and always ersprect all coaches, calling them "Coach" or Mr./Mrs. "last name"

- I will have sportsmanship as my #1 priority in all games and practices. 

-I will be a good teammate by having a strong commitment to excellence and by putting in hard work both at practice and in games. 

- I will work hard in school and maintain a C average/ level 2 average. 

- My partens can limit my partiicipation if I fail to do my best in school. 

- I will respect and obey my parents/guardians.

- I will respect reffees, and opposing players and coaches at all times. 

- If I  have an unsportsmanlinke event, It will result in my immediate removal from game or practice and a loss of playing time will result. 

- If i have an unsportsmanlike event that is the result of a poor interaction with an official, it will result in my removal from a game and a one-game suspension.

- Repeated or severe 1st time unsportsmanlike violations will result in expulsion from Warhawk Nation Youth Sports Tackle Football. 

- I will not use profanity. Abusive language or sursing will NOT be tolerated. 

-I will encouage my teammates in a positive way, regardless of circumstances. 

I will attend all scheduled games and practices. 

-If unable to attend a game, I will contact my coach beofrehand.

-Missing two consecutive practices, can result in loss of playing time. 

-I will respect, protect, and maintain my equipment, I will NEVER throw my equipment.

Fees are $175 and due no later than April 15th 2026

Players/partents are responsible in providing a mouth guard and cleats. 

Uniforms are included in registration fees.

Warhawk Nation Youth Sports Waiver/Release Form

Release of Liability for minor participants- READ BEFORE SIGNING. My child is allowed to participate in any way in the Warhawk Nation Youth Sports Flag/Tackle Football related events and activities, the undersigned acknowledges, appreciates, and agrees that:

The risk of injury to my child from the activities involved in these programs in significant, including the potential for permanent disability and death, and while rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist. 

1)FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE PARTICIPANTS, spectators, administrators, coaches, or others, and assume full responsibility for my child's participation. 

2) I willingly agree to comply with the program's stated adn customary terms and conditions for participation. If I observe any unusual significant concern in my child's readiness for participation and/or in the program itself, i will remove my child from the participation and bring such attention of the nearest official immediately. 

3) I myself, my spouse, my child and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Warhawk Nation Youth Sports Football; its administrators, directors, officers, officials, agents, employees, coaches, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used o conduct the event, WITH RESPECT TO ANY AND ALL LIABLIITIES, INCIDENTS, INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child's/ward's involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. 

4) I myself, my spouse, my child and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS all the above releases from any and all liablities incident to my involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE to the fullest extent permitted by law. 

I HAVE READ THIS RELEASE OF LIABLITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOULUNTARILY WITH OUT INDUCEMENT
