Hi, I’m Jasmine — a licensed cosmetologist based in Southwest Florida (Fort Myers). I specialize in leave-out quick weaves and custom glue-less closure wigs. This form is the first step in my process. It allows me to understand your needs and determine whether my services are the right fit before moving forward with a consultation or booking. A quick weave is an install where your natural hair is secured and protected underneath a protective cap + molding base, while a small portion is left out for blending. The extensions are applied onto the cap to create a smooth, stable foundation for styling and a natural-looking finish with your leave-out.

You may:

• Bring your own extensions

• Or purchase extensions directly from me

This service includes:

• A thorough shampoo and blowout to prep your natural hair

• Braid-down foundation

• Protective mold + cap placement

• Seamless track placement

• Face-framing layers

• Your choice of body-wave curls or beach-wave curls

Optional Add-Ons:

• Scalp treatment

• Deep conditioning

Requirements

• Minimum hair length: 5–6 inches all the way around. This ensures you have the best blended install.

• No pixie cuts unless they’ve grown out.

• Hair must be detangled.

• If you have a different styling idea, we can discuss it to see if it aligns with this install.

• All extensions must be approved by me before your appointment is confirmed.
