Community Health Ambassador (CHA) Application Form 2026

Thank you for being interested in volunteering as a Community Health Ambassador. Let's work together to raise health awareness in the community!

There are two categories for CHA recruitment:

1. Organization
   • You found out about this project through your organization (company/ NGO/ university/ college).
   • You are registered under your organization & will be representing your organization. 
   • Kindly ensure you enter the full name of your organization under "Associated Organization".
2. Individual
   • You find out about this project through other platforms (social media, family members, word of mouth, etc.)   OR
   • Your organization is not able to fulfil the minimum requirement of 10 registrations.
   • Kindly ensure you enter N/A under "Associated Organization".

You will be required to complete a one-day training course before volunteering commences. The date and time will be arranged soon.

We shall send you the training details via WhatsApp/ email if you are chosen as a Community Health Ambassador.

This authorisation shall continue to be effective until revoked by me. I understand that any misrepresentation of the information I provided may lead to a void of application. You can recheck your information that is entered through the email. For the process of approval, it might take one or two weeks. Kindly monitor your email inbox to get the result of application.
