Waiver Agreement
Laser Landing, LLC Assumption of Risk, Waiver, and Release of Liability (For Adult Participants and Parents/Guardians of Minors) In consideration of being permitted to enter, observe, or participate in any activities, games, events, or programs at Laser Landing, LLC (“the Facility”), located at 211 Piper Circle, Annville, PA 17003, the undersigned (“Participant”) agrees as follows: --- 1. Acknowledgment and Assumption of Risk I understand that activities at Laser Landing, LLC—including but not limited to laser tag, arcade play, birthday parties, group events, and general recreational use—can involve inherent risks, such as slips, trips, falls, collisions, equipment malfunctions, or actions of other guests. I acknowledge that these risks cannot be eliminated without jeopardizing the essential qualities of the activities. I voluntarily assume all known and unknown risks, including any injury, loss, or damage to myself, my child, or property that may occur as a result of my or my child’s participation or presence on the premises. --- 2. Waiver and Release (Adults Only) If I am 18 years of age or older, I hereby waive, release, discharge, and hold harmless Laser Landing, LLC, its owners, members, employees, contractors, affiliates, and representatives from any and all claims, demands, or causes of action for personal injury, death, or property damage arising out of or connected to my participation or presence at the Facility, whether caused by negligence or otherwise, to the fullest extent permitted by Pennsylvania law. --- 3. Parental/Guardian Acknowledgment (Minors) If I am signing as a parent or legal guardian for a participant under 18 years of age, I acknowledge and agree as follows: I understand and accept the risks described above on behalf of my minor child(ren). I consent to my child’s participation in activities at Laser Landing, LLC. I understand that under Pennsylvania law, I cannot waive or release the legal rights of a minor child to bring a claim for injury. However, I agree to indemnify and hold harmless Laser Landing, LLC from any claims or damages brought by or on behalf of my child, to the fullest extent allowed by law. I accept full financial responsibility for any damages or injuries caused by my child while on the premises. Names of Minor Participants: --- --- --- 4. Photography and Media Release I grant permission for Laser Landing, LLC to use photographs or video footage of myself or my child(ren) taken during participation for promotional or security purposes. I may revoke this consent in writing at any time. --- 5. Facility Rules and Conduct I agree to comply with all Facility rules, signage, and staff instructions. I understand that failure to do so may result in removal without refund. I am responsible for the conduct and supervision of minors under my care. --- 6. Severability If any portion of this agreement is found unenforceable, the remaining provisions shall continue in full force and effect. --- 7.
